1 2inch Mead Five Star zipper binder
2 Sets of plastic dividers with pockets for binder (10 dividers)
1 Ruler with inch and centimeter measurement
1 Protractor
1 Pair scissors
2 Red ballpoint pens
6 #2 pencils (no lead pencils)
1 Black Sharpie-fine point marker
1 Highlighter (please do not bring more than 1)
1 Eraser
1 Glue stick (1.4 oz)
1 Liquid glue (5 oz)
4 Single subject spiral notebooks, 70 sheets, college-rule
1 Folder
1 24 count box of crayons (please do not bring more than 24 count)

Emergency Supply List
In order to prepare our students for possible emergencies, Holy Name School is taking steps to comply with the Archdiocese Emergency Model for Schools. With that in mind, parents will be required to send the following items with their students at the beginning of each school year:
2 high energy bars (granola, etc.). Avoid any containing peanut butter.
1 fruit cup, not from refrigerator section.
1 small package unsalted crackers or wafers (not larger than 4x4x2 in.)
1 four oz. size Jell-O gelatin cups – not pudding
2 fruit roll-ups
2 small canned juice (5.5 ounce) – no boxed juices
1 pint (16 oz.) plastic bottled water
2 small pocket sized Kleenex packets
A note and/or photo from family reassuring to child
A small book, puzzle, and/or toy, which will fit inside bag
The above items need to fit in a one-gallon zip lock bag, clearly labeled with your child’s name. Please take note that these bags will be kept in the classroom, not in a refrigerator. Do not include any items that require refrigeration. Items should have an expiration date past May 30, 2017. This bag will be sent home at the end of the school year.
