Nomination Form for Individual Awards

Submission Guidelines

Objectives of Awards– To recognise the dedication and high-level of service provided by a collection crew involved in either waste collection, recycling or both.

The judges will be looking for a commitment to providing a high level of service which can be measured in terms of consistency of service, the quality of provision, safety records and additional factors, such as, where service goes beyond the normal level. The entry is likely to include testimonials and photographs.

Award Categories – There are two award categories, “Outstanding Frontline Operations Crew (Individual)” and “Outstanding Collection Crew (Team)”, which are applicable to employees across the different waste management sectors.
Evaluation Criteria–Nominations will be evaluated based on the following criteria;
Outstanding Frontline Operations Crew (Individual):
Works in Singapore; and
From a local waste management Company.
Outstanding Collection Crew (Team):
Works in Singapore; and
From a local waste management Company.
Assessment Process – Nominations will be subject to a rigorous selection process by the judging panel. The panel includes:

National Environmental Agency (NEA);
Workforce Singapore (WSG); and
Workplace Safety and Health Council (WSHC).

Submissions:

Outstanding Frontline Operations Crew (Individual):

Each company is allowed to submit a maximum of 2 entries.

Outstanding Collection Crew (Team):

Each company is allowed to submit a maximum of 2 entries.

Submissions must include:
Completed Nomination Form;
A PowerPoint presentation showcasing the Crew at work together with photos (max 5 images). Please use the attached “Powerpoint Presentation Template” and
No logo or company name shall appear in any of the PowerPoint submissions. Name of companies shall only be indicated in the entry forms. Please specify the category that the entry is submitted for in the presentations and entry forms.
Awards

Outstanding Frontline Operations Crew (Individual):

There will be 3 winners; Gold, Silver and Bronze.

Outstanding Collection Crew (Team):

There will be 3 winners; Gold, Silver and Bronze.

The nomination form and all accompanying materials must be submitted in English.
Nominations should be via the nominee’s respective Company only. Self-nomination is not encouraged.
Once nominations have been submitted, neither the nominator nor nominee will be able to make any modifications. The Awards Organising Committee reserves the right to disqualify any nomination deemed to be incomplete or inaccurate.
Each entry is only allowed to be submitted once.
The Awards Organising Committee may request for additional details and/or arrange for interview where necessary.
Results will be announced at the “WasteMET Asia Symposium 2017 – “Industry Awards Dinner” dated 19 Oct 2017.
Nomination entries for the Waste Management Industry Excellence Awards2017 will be open from18 Jul 2017 to18Sep 2017. Any nominations submitted after 18 Sep 2017 will be considered as void.
