Paperwork Submitted Checklist

NAME:

Where a box has been ticked, it indicates that the document was provided at interview. If a box IS NOT TICKED the information is outstanding. You must provide any missing documentation before 12th July in the year of entry to the programme. Candidates that do not return all documentation on time may have their place withdrawn regardless of academic results.

For those applying to healthcare courses ONLY, there are forms that you were not required to complete at interview that you must complete now. This includes health and the on-line police clearance application which constitute non-academic conditions of offer.

1.  For all candidates

Student Entry Information – if the box is not ticked, please read and sign this document and return to Admissions as soon as possible. o

For Biomedical Science, Healthcare Science and Radiography applicants – please cut and paste the following link into a web browser and this will take you to the portal page where you can down load the student entry information form.

Photographic ID – if the box is not ticked, please send one of the following forms of ID: passport; driving licence (both photo-card AND paper counterpart); national identity card. Please do NOT send originals to us in the post. We can only accept certified copies of the ID documents (verified by an official body, your current school, college, or university). o

The name you have put on your UCAS form does not match the name on the documents you have provided. You will be registered as:

You do not have to provide us with anything unless you wish to be registered under a different name. If you wish to use a different name, you need to send further documentary evidence with your name as you would like to be registered. NB Only a birth certificate, marriage certificate, statutory declaration or deed poll are acceptable. o

Certificates – If the box is not ticked, please provide the following outstanding certificates:

You should NOT send original certificates – we can only accept certified copies (verified by an official body or your current school, college, university). If you do not have some of your certificates you must ask the Examining Board for further copies. PLEASE NOTE that we cannot accept certified copies of “Statement of Results slips” as these are provisional and not official documents. Alternatively, you may ask your school/college/university to provide an official letter confirming your grades. This must be on headed paper and signed. o

2.  For candidates applying to healthcare courses only (all Medicine degrees, Physiotherapy, Healthcare Science, Diagnostic and Therapeutic Radiography, Foundation Degree in Paramedic Science).

SGUL Criminal Records Disclosure – if the box is not ticked, please read and sign this document and return to Admissions as soon as possible. o

The documents listed below would not have been required at interview stage but must be completed now. Health and police clearance constitute non-academic conditions of offer.

Health Questionnaire - Please print the health questionnaires from the Portal as directed in the main information regarding your offer. There are sections to be completed by you and your GP. Please make an appointment to see your GP upon IMMEDIATE receipt of this form to ensure that there are no delays to submission. The forms must be submitted in the envelope provided to you with your offer letter and should be returned direct to the Admissions Officer for your programme. DO NOT send health forms direct to the Occupational Health department. o

3.  For candidates applying to Medicine degrees only

If the box is not ticked, this means that you have not provided us with complete official evidence of all of your work/relevant experience. Please send in certified copies of reference letters for the placements you listed on the form you brought to interview. We are not able to accept e-mailed/scanned references. References should be presented in the format indicated in the information we sent to you inviting you for interview, detailing exact dates and hours worked, as well as a brief outline of the duties you undertook. These letters must be on headed paper and signed, with the referee’s contact details. o

4.  For candidates applying to all Medicine degrees, Foundation Degree in Paramedic Science, Healthcare Science and Biomedical Science only

You should by now have applied for student loans and/or grants. If you have not done so, you should contact your Local Education Authority (LEA) for forms.

5.  For candidates applying to Physiotherapy and Diagnostic and Therapeutic Radiography only

You should have been informed by the NHS student grants unit of your bursary application reference number. If you have not yet received this then you should contact the bursary unit directly.
