Elementary School- Child Study Team Meeting

Daisy is a 4th grader and is struggling with academics. The teacher met with her grade level team and the counselor to discuss the situation. The teacher shared that Daisy is sometimes disengaged in class and is reluctant to participate in group activities. eCST records show that she failed TAKS reading in 3rd grade. Daisy’s parents are also concerned about her lack of academic progress. The beginning of the year universal screener indicates Daisy has basic decoding skills but lacks fluency and comprehension. Her reading fluency rate is currently a 95wpm. Based on this information, the team developed the following reading intervention plan:

Reading goal: Daisy will read a 4th grade reading passage at a rate of 120 wpm.

Interventions:Daily review of high frequency words using flash cards; partner and choral reading during small group; review of word patterns and families during guided reading

Secondary School- Campus Leadership Meeting

It is the second month of school at Happy High School. During a faculty meeting teachers expressed concern regarding the number of tardies that are occurring. Administrators noted they have also seen a number of students in the hallway after the bell has rung. At the next Leadership meeting, the principal and administrators reviewed the following data:

Unexcused tardies(10/5/12 through 10/20/12):

9th grade / 10th grade / 11th grade / 12th grade
199 / 115 / 83 / 110

Total: 495 unexcused tardies

Notes from faculty meeting:

  • Many students coming in late at the beginning of the day
  • Students don’t seem to have a sense of urgency regarding getting to class on time
  • Students lingering in the hallways, talking to peers
  • Groups of students clustered together
