Dear Parents or Guardians,

As Part of our cultural study of Latin America and Spain we will be taking an educational field trip to see a performance by the Hispanic Flamenco Ballet Co. in West Hartford, CT on Monday, Nov 6th 2017. The bus will depart from the school parking lot at 7:30 am and we expect to return home by approximately 2:30 pm. PLEASE BE AT THE SCHOOL READY TO LEAVE BY 7:20 am.

Your child needs to bring a bag lunch since we won’t have time to buy food anywhere.

If your son/daughter requires medication during the day, you need to fill out a medication form. Please see the school nurse. Confirm that emergencies are provided for (asthma, bee sting, etc.) if necessary for your child.

Please consider accompanying us. If you are interested in being a chaperone specify so at the bottom of this form.

We have already discussed the proper behavior for the bus and the performance. Please review this with your son/daughter. Any students with D or F grades in any classes as of Monday Oct 30th will not be allowed to go on the trip. They will not be reimbursed for their $17 for the ticket. There will be back up students if someone gets sick or can’t go, but the reimbursement isn’t guaranteed.

If you are absent the day of the Field Trip the ticket can’t be refunded. (unless one of our backups can take their place)

Please complete and return this form along with $17.00 by Monday Oct 23rd, 2017 to save your child’s spot. You can make checks out to Hinsdale High School. There are a few financial hardship scholarships available please see Ms. Sayán to inquire.
