ELEMENTARY SCHOOL ORDER FORM

Dear Educator:
In preparation for the Diocese of San Jose High School Placement Testing Program (HSPT®) test dates scheduled for January 10, 2015 and January 24, 2015, please be advised that all Diocesan elementary schoolshave the opportunity to order any of the following, optional HSPT®score reporting services for your students whowill take the HSPT.
Please note that we have assigned a unique three-digit code number for each Diocese of San Jose elementary school, attached hereto for your review. Diocesan high schools will provide each student with a code list on test day, and students will entertheirelementary school code number on their answer sheet before testing begins. From these coded answer sheets, we are able to provide your student scores.
The purchasecost of any of the optional scoring servicesbelow shall be paidby your school, via invoice forwarded to you after testing has been completed in February 2015. If you wish to receive any of the following score reporting services, pleaseplace your order by completing this order form and returning to STS no later than December 22, 2014, via facsimile to 1-866-766-8054, or by mail using the enclosed envelope.
The HSPT® Alphabetical List Reports organize students in alphabetical order and provide cognitive skills quotients (CSQs), standard scores, national and local percentiles, as well as their grade equivalents for the basic skills subtests – reading, mathematics, and language (two copies). This is available for $0.98 per student.

The HSPT® Group Summary Statistical Report, which summarizes overall group performance on all subtests and overall group performance on each subtest. This report is available for $0.98 per student.

The HSPT®Performance Profile is an individual diagnostic profile generated for each student, which reports his or her scores, identifies corresponding national and local norms, and gives information about performance on skills within each subtest (2 copies). The Performance Profile Group Summary is included at no additional cost. This scoring service is available at $1.60/student tested.

The HSPT®Individual/Group Item Analysis Reports are provided for the three basic skills subtests – mathematics, reading, and language for $2.54 per student. The Individual Item Analysis Report provides performance information for each student on an item-by-item basis and relates it to a comprehensive outline of specific skills. For each item, a “+” represents a correct answer and the student’s actual answer is displayed for a wrong answer (2 copies). The Group Item Analysis Report is included at no additional charge with the Individual Item Analysis Report, but it may be ordered without the individual data if desired. It provides a summary for the group average, and is available for $2.15 per student (2 copies).

Please be advised that each of the above reports are available as PDF files, whereby you will receive one PDF file for each report ordered. PDF reports not only reduce shipping costs, but also allow you to quickly locate a student’s results within the file using the “find” command. Pricing is the same for paper or PDF reports. If you wish to forego paper reports for this PDF reporting option, please check the line below and supply an email address to which the reports will be sent.
