NURSING CARE ASSISTANT/HEALTH UNIT COORDINATOR

- Performs patient care tasks under the direction of RN/LPN
- Demonstrates technical proficiency in use of patient care information system, orders on electronic medical record (EMR) task list, verify orders to request diets, treatments, equipment and diagnostic/laboratory procedures.
- Schedules return appointments upon patient discharge per protocol
- Prepare and upload paper documents into the EMR
- Collects patient data and records in electronic medical record (EMR)
- Recognizes patient data that is not within unit clinical standards and/or new abnormal changes in patient condition and reports to supervising RN within specified time frame
- Communicates patient information promptly, with 100% accuracy
- Responds to patient /family requests & needs promptly (i.e. assistance to BR, turning, I&O)
- Proficient in use of telephone, paging system, unit/ nurse call systems and PIN resources
- Demonstrates Safety Awareness/Preparedness and Adheres to patient safety principles:
- Collaborates with charge nurse to respond to emergency situations on the unit using downtime and manual backup system as needed
- Actively participates in patient/unit safety initiatives

- Anticipates needed equipment, supplies and monitoring for patient population
- Actively seeks avenues to improve process in providing patient care and enhancing patient outcomes
- Participates in analysis/problem solving of actual or potential safety issues.
- Audits and promotes unit compliance with regulatory standards
- Participates with team in design of patient care with supervising RN
