Instructions for completing the application form

1Applicants must use the application form of the Foundation (A-4 format, 5 pages in total including 1 page with contact details). Information must be entered in the designated fields.

2Application forms must be filled in using Microsoft Word, for Japanese text applying typeface Mincho, font size 10.5, and for English or German text applying typeface Times New Roman, font size 10.5.

3Applicants must sign in the space marked "Signature."
Applicants must complete all required information fields of the application form. Only necessary documents are permitted as attachments.
Please check that no information item straddles two pages. Please note that an application will not be accepted if either the number of pages or the application format has been changed.

4Application forms must be submitted by e-mail as PDF attachments. Received application forms will not be returned.

5Please observe the following points when entering the above information.

- Research expense plan

In the space for the breakdown of the total cost of research, please enter specific amounts, content descriptions, and the specific amount of aid of the Foundation.

- Research funding plan

In the research funding plan, please enter specific information. Parallel requests at other intuitions are permitted. Please approach the secretariat promptly if a concurrent aid application with another institution is pending or is subsequently granted.

Important points concerning applications

  1. Reports pursuant to item 12 of the application guidelines will not be returned.
  2. Applicants agree that all copyrights in all countries (including rights pursuant to articles 27 and 28 of the Copyright Act of Japan and similar rights provided in each country’s laws and regulations) related to reports are transferred and solely belonging to Yamaoka Memorial Foundation.

Applicants agree that prior to the release of report content to other media, the consent of the Foundation must be obtained and references as directed by the Foundation must be additionally noted. Applicants agree to refrain from making announcements to other media prior to the public release on the website, etc. of the Foundation.
In unavoidable cases, the Foundation must be contacted in advance and the instructions of the secretariat must be followed.

  1. With respect to the participation in the symposium pursuant to item 12 of the application guidelines, the rights to applicants' likeness are transferredto the sponsor.
  2. Personal information provided in connection with applications will be exclusively used for matters incidental to notifying selection results and will be treated appropriately in accordance with the privacy policy of the Foundation (Personal Information Protection Policy
  3. The names of researchers and their affiliated institutions will be made public.
