Statement of Purpose
This is a Statement of Purpose for St Melor House Surgery which sets out the following information:
- The full name of the service provider and of any registered manager together with their business address, telephone number, and where available electronic mail addresses.
- The legal status of the service provider.
- Details of the locations at which the services provided for the purposes of the regulated activity carried on
- Our aims and objectives in carrying on the regulated activity.
- The kinds of services provided for the purpose of carrying on of the regulated activity
- The range of service user’s needs for which those services are intended to meet.
St Melor House Surgery is a General Practice Partnership open to all patients living within our practice boundary in Amesbury and the surrounding area. We also will accept patients who do not live in the area and can be registered as an “out of area” patient.
We are a General Medical Services (GMS) practice offering primary care services for our patients.
We help patients to manage their health and prevent illness. Our GP’s assess, diagnose, treat and manage illness. Our GP’s act as a patients advocate, supporting and representing a patient’s best interests to ensure they receive the best and most appropriate health and/or social care. The GP’s provide the link to further health services and work closely with other health care colleagues. They arrange hospital admissions and referrals to other services and specialists, and provide a link with secondary and community services.
St Melor GP’s are supported by an excellent team consisting of a nurse practitioner and experienced practice nurses with expertise in the management of chronic diseases – diabetes, respiratory and chronic heart disease.

Our Services
The GMS services provided by the GPs are :
Our core services are GP consultations, asthma/COPD clinics, diabetes clinics, coronary heart disease clinics/hypertension clinics
Additional services include cervical cytology screening, contraceptive services, child health surveillance, maternity services, minor surgery procedures, vaccinations and immunisations.
Enhanced services include – contraceptive coil fitting, contraceptive implant fitting, childhood vaccination and immunisations, flu immunisations
Other services: ear syringing, medication reviews, stop smoking support/clinics
Travel advice, men’s health, women’s health, ring pessary replacement,
Non NHS services:
Our practice also provides services which are non NHS and are paid for by the patient. These services include: Insurance claim forms, Non NHS vaccinations, private sick notes, pre-employment and HGV/PSV medicals, vaccination certificates.
Chaperones – we have a chaperone policy in place.
Access to patient information – all patient information is considered to be confidential and we comply fully with the Data Protection Act. Employees have access to patient information according to their role and have signed a confidentially agreement. Staff have access to Caldicott guidelines and information is displayed prominently.
Complaints – we have a complaints policy in place with a complaints leaflet available if needed.
