Changes compared to the version used in the initial validation studies (Van der Zee et al., 2010, 2011; Post et al., 2012):

- merging items on housekeeping and chores in/around house

Motivation: both items overlap and to make space for additional items

- splitting up satisfaction with work and satisfaction with housekeeping (S1/S2)

- splitting up satisfaction with outdoor activities into 3 separate items (S4/S5/S6)

Motivation: to improve the comparability of items on restrictions and satisfaction

- removing items on satisfaction with communication ability and self-care ability

Motivation: these items concern activities, not participation

- adding item on restrictions with transportation (R3)

Motivation:to improve the comparability of items on restriction and satisfaction.Although transportation might not be considered participation, the clinical relevance of these items is high and using transportation facilities will never be part of the “activities USER”, so that it was decided to keep it in and to add restrictionsitem.

- adding item on restrictions in relationship with partner (R8)

Motivation: clinical relevance and comparability of items on restriction and satisfaction

- several minor edits
