A. Pest Control Contract
1. Requirements
a. The Contract shall become effective on (insert date) for a period of
twelve months. The Contract shall apply to all real properties owned
by the (insert Authority). These properties consist primarily of multiple
family dwelling units, but not limited to, storage sheds, offices, garages
and other common areas incidental to the operation of the Housing
Authority. Specifications can be picked up at (insert address)
starting on (insert date). Deadline for submission: (insert date
and time).
b. Purchaser's Name- The Contract shall name as purchaser, (insert
Authority).
c. Service/Coverage- The pest control company agrees to provide
control of the following pests: Brown recluse spiders, black widow
spiders, fleas, ticks, bees, clothes moths, house crickets, millipedes,
earwigs, wasps, centipedes, yellow jackets, carpenter ants, house
ants, pharoch ants, silver fish, rats, mice, smoky brown roaches,
american roaches, oriental roaches and german roaches.
d. Service Location- Pest control services will be provided for:
e. Service Schedule- The pest control company will service the (insert
Auhority)'s premises one (1) time per month. Service time schedule
will be arranged by the (insert Authority) and agreed upon by both
parties. In addition, the pest control company will provide additional
service as deemed necessary by the (insert Authority) at no additional
charge. Such service visits will be made promptly when requested by
the (insert Authority).
1. The pest control company will provide an initial flush of all units.
If after the initial flush, a problem exists, the pest company will
reflush within seven (7) days.
f. Service Technician(s)- The pest control company agrees to allow the
same service technicians who performed the initial spraying to continue
regular spraying throughout the life of the Contract.
g. Payment- In consideration for the service to be provided, the (insert
Authority) agrees to pay the pest control company on a bi-monthly
basis. Payment will be made upon receipt of invoice.
h. Money-Back Guarantee- If, during the regular scheduled service, the
(insert Authority) is not satisfied with the requests for service
maintenance, the pest control company will work to correct the problem
to the satisfaction of The Housing Authority or will return the Housing
Authority's previous monthly payments.
i. Licenses/Certification- All applicators and technicians who will be
performing the structural pest control work must be licensed and
certified by the Texas Structural Pest Control Board. Proof of this will
have to be submitted with the Contract.
j. Materials- Pesticides must be registered with the United States
Environmental Protection Agency and the Texas Department of
Agriculture. All pest control work will be performed in a safe manner
and in accordance with the procedures specified on the label. A list of
the chemicals used will be provided to the Authority.
k. Insurance- The pest control will furnish the (insert Authority) a current
certificate of liability insurance together with the bid.
l. Equipment Replacement- The (insert Authority) will not be held liable
to repair or replace any pest control company equipment lost, damaged
or destroyed on its premises.
m. Special Instructions- (I) It will be required that the contractor posts
notices at each unit prior to the treatment. In addition, that an initial
service of all false bottoms of cabinets non-masonry wall, behind
plates of light switch and electrical outlets, and attics be injected with
powder insecticide every quarter. (II) After the initial spraying of all
units and facilities, treatment of all units and facilities will occur every
month.
Taxes and Fees- The (insert Authority) is a tax-exempt organization.
Debarred, Suspended and Ineligible Contractors- The bidder certifies by submission of a bid that is not a debarred, suspended or ineligible contractor by any Agency of the Federal or State Government. (No bid received from a debarred, suspended or ineligible contractor will qualify for an award.) Enclosed form HUD-2992 is required to be included in your bid proposal.
