Position Summary:

The Patient Navigator will be based in the School Based Health Center. They will provide patient navigation to the students and their families. This will include assistance in making appointments in the School Based Health Centers for Behavioral Health, Dental, Primary Care and Reproductive Health. The Patient Navigator will also be responsible for following up on Patient Care, and maintaining a positive communication between the students and families with the School Based Health Centers.

The Patient Navigator must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's individual needs. The Patient Navigator will provide a safe and confidential environment for patients, families and personnel.

SBHC Patient Navigator Essential Functions- On site position

SBHC Patient Navigation – Filling appointments at SBHC sites

·  Identify a student from the enrollment spreadsheet

·  Verify Insurance

·  Pull immunization history from CIIS and Power School

·  Log TDAP, HPV, Meningococcal and Varicella dates

·  Identify last known well child exam or sports physical with SCCC or SBHC

·  Communicate effectively with students and families for appointment notification, necessary follow up appointments, referrals to other providers, and completion of necessary patient paperwork.

SBHC Practice Management

·  Communicate effectively with students, staff and families for appointment notification, necessary follow up appointments, referrals to other providers, completion of necessary patient paperwork.

·  Insurance- Verify the insurance eligibility of patients, verify demographics of patient, verify accuracy of patient information in Aprima. Refer uninsured patients to Medicaid Enrollment opportunities. (Public Health or FIRC)

·  Prep appointments for the following day and confirm appointments if appropriate.

·  Take payment and post to Super Bill

·  Run and Print Daily Deposit

·  Deposit delivery

·  Help CA with Scanning

·  School Newsletters

·  Other duties as assigned

Appointment Tasks:

·  Call patient from class/ check in patient

·  Consent and enrollment verification

·  Delivery of the eSHQ and PHQ to the patient.

·  Log new immunizations into Power School

·  Make sure CIIS , Power School and Aprima match

Immunization Responsibilities

·  Primary or Secondary point of contact for VFC program

·  Log vaccine refrigerator and freezer temps twice daily on the temp log sheets

·  Print immunization history from Power School and CIIS for scheduled appointments

·  Other duties as assigned

SBHC Patient Navigator School Responsibilities- Only required for those persons supporting the District Health Aid position

1.  Triage of student medical needs

·  Student sign in

·  Medicine Administration- must be District trained

·  Ice

·  Rest and Water for Illness

o  Take Temperature

o  Call and receive parent permission prior to dismissing student from school

o  Call attendance office to relay verbal permission from parent

o  Inform student to sign out with Bonnie

2.  Clean clinic beds

3.  Enter each student visit into Power School

4.  Attend Summit Health Leaders meetings

5.  Enter new immunization in Power School and Match with CIIS

6.  Send immunization requirements home to all non-compliant students

7.  Send hearing and vision letters to 9th grade students

8.  Complete incident reports

Education/Experience:

Certificates and Licenses:

OSHA required courses for positions, CPR.

Work Environment:

The work environment is in a healthcare setting and can be fast paced and involve working directly with patients and healthcare professionals. Many of our patients with chronic disease have a minimal level of health literacy and a significant number of barriers to accessing health care. The individual may also work in a clerical setting.
