GRANT REPORT NARRATIVE
Please respond to all of the questions in this section using a maximum of 4 typed pages:
- Briefly outline your original goals and objectives, as stated in your proposal and the progress you have made toward these goals and objectives. What activities led to meeting these goals and objectives? How were these objectively monitored and measured?
- Describe the population served or community reached during the grant period. Be specific, such as, number of individuals served, race/ethnicity, gender, geographic location, etc.
- What were the results/outcomes of your funded program, either positive or negative. What obstacles did you encounter while implementing this program, and how did you address each one? What changes did you or will you make based on these results/outcomes?
- What did you learn because of this grant that will assist you or others in serving disadvantaged youth and families? What plans, if any, do you have to share your results or findings with other organizations?

FINANCIAL REPORT

Please Note for Financial Report items 2 and 3: Use of grant funds for purposes other than those noted in your Agreement letter requires authorization from the Foundation. Surplus funds at the end of the grant period must be returned to the Foundation unless other arrangements have been made as authorized by the Foundation.
