It is your legal responsibility to inform Care and Social Services Inspectorate Wales (CSSIW) about certain changes that take place in your registered service. You can use this form to tell us what the change is and give us the details we need to make sure that your business continues to operate within the law.

The changes you must tell us about: / Regulation / TickIf you or any staff you employ are convicted of any criminal offence in Wales or elsewhere. You must tell CSSIW the date and place of conviction, the offence of which convicted and the penalty imposed. / 10(1)
If you are charged with any offence in respect of which an order may be made under Part ll of the Criminal Justice and Court Services Act 2000 – you must give notice in writing to CSSIW of the offence charged and the date and place of charge: / 10(2)
If a registered person dies, either the surviving registered person orthe deceased’s personal representatives must notify CSSIW in writing within 14 days / 11(6)
Any changes to your statement of purpose – whenever practicable, at least 28 days before the change is due to take effect / 15(4)(b)
If you make changes to your behaviour management policy you must notify CSSIW within 28 days. / 23(4)
Any information which renders you disqualified under regulation 11 of the Child Minding and Day Care (Disqualification) (Wales) Regulations 2010. These can be found at / 11

Any serious injury, or serious illness or death of any child or any other person on the premises. / 7
If you, or anyone living, working or employed on the premises, are accused of committing serious harm to a child or any abuse is alleged to have taken place on the premises. / 8
Any other event which may affect your suitability to look after children or the suitability of any person living, working, or employed on the premises to be in regular contact with children. / 9
Any other significant event which is likely to affect the welfare of any child on the premises. / 10

Failure by a Registered Person/Responsible Individual, without reasonable excuse, to comply with any of the above notification requirements would be an offence for which, on conviction, the person would be liable to a maximum fine of level 5 on the standard scale.
