Audition Application Form

(Please note - however, places may be limited and we cannot guarantee an audition on the day)
This is an opportunity for individuals or groups, aged 8-19 years(up to age of 25 for children with disabilities) to display their artistic talents of singing, playing in a band, dancing or any other performing art forms. Auditions will take place on the following dates, times and locations:

  • All acts must be safe and appropriate for family viewing. No profanity, sexual behaviour, nudity, violence, derogatory or offensive language.
  • Any acts requiring backing tracks/ music will be required to bring a compact disc to auditions containing the track.
  • Please have a 2 minute prepared piece. Performance pieces must be at least 2 minutes long and no longer than 4 minutes.
  • The closing date for application forms to be submitted are 29th April 2018.
  • Any applications submitted after the closing date will not be considered.
  • All performers must live, work or study for a minimum of 12hrs in Sandwell and are aged 8 – 19 (up to the age of 25 for children with disabilities).
  • Any performer under 16 must be accompanied by a parent/ guardian on audition day.
  • If your act has appeared in the final for the last 2 consecutive years you are not permitted to participate this year.
  • If you are a previous winner of SHAPE Your Talent, you cannot enter the competition again unless it is in a different age category.
  • Any soloist performer can perform in a group as a separate act but must submit an application form for each act.
  • If you are a member of a group, you are unable to participate as a member of any other group, but can participate as a soloist.
  • If you are a soloist performer and would like someone older to provide backing music via an instrument, this is permitted however only you as a performer will be judged.
  • An application can be made for one venue only. If the act is not successful in the heat applied for then they can go to one other heat to audition, providing there is space available.
  • Proof of age: 16 and over (Birth Certificate, ID, passport, anything with official birth date)
  • It is expected the event will attract media and/or public interest, which may involve photography and video. By entering the contest you are consenting to such publicity.
  • The Council and partners will be filming/ taking photographs of acts to be used in publicity and on social media (including on YouTube) and by entering the contest you are consenting to such publicity.
  • All personal details and/or information given in your application either in this form or otherwise must be truthful, accurate and in no way misleading.
  • You can be disqualified if: False information submitted, crude or inappropriate talent act or behaviour. Sandwell Council’s decision is final.
