These posts will be based at the Emergency Department, RoyalAlexandraHospital (RAH), Paisley (annual attendance 76,000) and has clinical duties at both the RAH and Inverclyde Royal Hospital (IRH), Greenock (annual attendance 35,000).
The posts would be appropriate for experienced doctors and are Locum Appointments for Training (LAT). The candidates should have additional training in other specialties relevant to Emergency Medicine and have experience equivalent to ST3 level or above. The successful applicants will work on the middle grade rota. The posts will commence in November 2012 be for a period of 6-12 months.
The medical team consists of 14 consultants, 3.5 specialty doctors, one associate specialist and 26 doctors in training grades. There are excellent educational facilities at both hospitals and study leave is actively supported. There is a regular teaching programme in place and all medical staff are well supported by enthusiastic consultant staff.
The successful candidates would be expected to provide support and supervision of junior medical staff, see and assess both major and minor presentations and actively be involved in the running of a busy district general Emergency Department which has a large number of paediatric attendances.
These posts hold Educational approval from the Postgraduate Dean and will be Locum
Appointments for Training or Clinical Fellows dependant on experience. If appointed
as a Clinical Fellow the post will not be recognised for training.
To practice medicine in the UK you need to hold both GMC registration and a licence to
practice.

EMERGENCY MEDICINE
INTRODUCTION
At present, there is a team of 14 Emergency Medicine Consultants providing an area wide Emergency Service. In addition the RoyalAlexandraHospital provides support to Emergency Nurse Practitioners at the Vale of Leven Hospital.
The town of Paisley, situated less than 10 miles to the West of Glasgow, is known world-wide for its historical contribution to the textile industry. Although its links with the industry are now a thing of the past, Paisley remains the largest town in Scotland. It is unique in terms of the ease of accessibility to the cultural and culinary delights of Glasgow, as well as to the scenic and recreational attractions of the West of Scotland.
There are excellent road and public transport links with the rest of Scotland and with the progressive upgrading of the main road route to England, journeys to locations south of the border are becoming ever easier. Glasgow Airport (average flight time to London Heathrow - 1 hour) is located only 4 miles from Paisley and has excellent scheduled links to the other major airports in the British Isles and Western Europe.
Property prices in the area are naturally dependent upon type and location. Generally, however, prices in the centre of Glasgow are the most expensive but drop markedly in the outskirts and surrounding areas despite the ease of access. There are a number of excellent schools and further education facilities, with Glasgow offering 3 universities in addition to the University of Paisley.

GENERAL INFORMATION
RoyalAlexandraHospital, Paisley. (RAH)
The town of Paisley is situated less than 10 miles to the West of Glasgow and 4 miles from GlasgowAirport (average flight time to London Heathrow 1 hour). The RoyalAlexandraHospital provides an extensive range of acute health services to the Renfrew District and beyond, with a population of 220,000 mainly concentrated in and around Paisley.
The Hospital first opened in 1986 and has a current bed complement of 520. There are also facilities on site for General and Geriatric Psychiatry. In any one year the hospital treats nearly 30,000 inpatients, over 103,000 outpatients and day cases, and there are some 2,400 births in the Maternity Unit.
The Emergency Department is situated in a purpose built module, which includes Fracture/Orthopaedic Clinic facilities. The Intensive Care Unit is located on the floor directly above the Emergency Department.
Annually the Department treats approximately 76,000 new attendances and 1,200 head injury admissions to Emergency Department beds. In addition approximately 2,800 patients per annum attended the weekly Soft Tissue Clinic.
The major specialties are General Medicine, General Surgery and Urology, Anaesthetics, Gynaecology, Obstetrics, Paediatrics, ENT, Ophthalmology, and Orthopaedic Surgery. The Radiology Department – with dedicated Emergency Medicine X-ray, Ultrasound, MRI and CT Scanner (available 24 hours) – is adjacent to the Emergency Department. There is a 24 hour laboratory on-call service for Haematology, Microbiology, Blood Transfusion, and Biochemistry.
There is also a Helipad in the Hospital grounds with direct access to the main Hospital building.
InverclydeRoyalHospital, Greenock. (IRH)
InverclydeRoyalHospital is a modern, 450-bed DGH in Greenock with magnificent views over the River Clyde and beyond. The Hospital serves a population of around 125,000 in Inverclyde, Largs, Bute and the CowalPeninsula. All major acute specialties are represented within the hospital, including General and Vascular Surgery, Urology, Orthopaedic Surgery, ENT, Ophthalmology, Gynaecology, Obstetrics, General Medicine, Rheumatology and Clinical Haematology. There is also a geriatric assessment unit and a stroke unit. Obstetric, ENT and paediatric services are centralised at the RoyalAlexandraHospital.
The Emergency Department is situated on the ground floor in a purpose built facility. The department has a varied case mix with 35.000 annual attendances and approximately 1500 return patients to the Soft Tissue Clinic. The department is well supported by the adjacent Radiology department and on-site laboratory facilities.
The Hospital has an active post-graduate education centre consisting of a well-stocked library, lecture theatre, four seminar rooms and reception area. The lecture theatre has seating for 80 with comprehensive audio-visual facilities, including live linkage to the main operating theatres plus the MATTUS link up to the Colleges of Edinburgh and Glasgow as well as the teaching hospitals in Glasgow, Edinburgh, Dundee and Aberdeen. We have also recently established a link via satellite with a European teaching system. The library has a bank of computers with easy access to the Internet for literature searches etc.
The standard of Primary Medical Care within the catchment area of all Hospitals is generally excellent, and major Regional Specialties are available in Glasgow. These include Plastic Surgery, Burns Unit, Maxillofacial Surgery, Cardiothoracic Surgery, Vascular Surgery and Neurosurgery.

The above staff are divided between the emergency departments based on the individual departmental workloads.
DUTIES OF THE POST
All experienced trainees will provide an integrated middle grade rota with other medical staff including EM STs, Specialty Doctors and an Associate Specialist.
Clinical duties will be divided between the RAH and IRH. It is anticipated that the majority of shifts will be on duty at the RAH department with less frequent commitment to IRH. Clinical work will involve a combination of emergency department work, ward rounds, clinics, teaching and supervision.
A commitment of approximately 4 in 9 weekends is anticipated. This is a band 1A 48 hr rota.
Clinical duties will include :
- Reception, resuscitation, diagnosis and initial treatment of all emergencies.
- Appropriate referral of patients to hospital beds, regional specialties, outpatient clinics, or back to the community.
- Organisation and supervision of follow-up clinics.
- Organisation and supervision of patients admitted to the observation beds.
- Organisation and supervision of minor procedures (including the manipulation of fractures and dislocations) in the emergency department.
- Organisation and supervision of pre-hospital treatment of specific patients out with hospital as requested by the emergency services.
- Planning for major incidents and the management of such within the emergency department, including the organisation and supervision of a mobile medical team.
Audit, Administration and Management duties will include :
- Organisation of, and involvement in, clinical audit projects.
- Involvement in the administration of the emergency departments.
- Involvement in the management of the departments’ as the need and opportunity arises.
Teaching duties will include :
- Clinical teaching of junior medical staff within the emergency department.
- Organisation of, and involvement in, formal in-service training of medical staff in the emergency department.
- Resuscitation training for appropriate groups of hospital staff.
- Participation in training/education events for other hospital medical staff as appropriate.
- Clinical teaching of medical students and extended training for ambulance paramedical staff.
- Participation in nurse education programmes as required.
Communication responsibilities will include :
- Effective communication and liaison with all medical and service departments in the Hospital, the social work department, and hospital management.
- Effective and timeous communication with general practitioners, including verbal and written communication on cases seen in the department.
- Effective communication with specialists in other hospitals.
- Communication with other agencies including the district nursing service and the emergency services.
Hours of Work
The appointee will join the middle grade rota. A full shift system operates. The appointee is contracted for 48 hours and the post carries a 1A banding, the banding supplement associated with this rota may be subject to change.

Candidates out with the Trust will be required to complete a confidential medical
Questionnaire following which, attendance may be required at the Occupational
Health Service.

TERMS AND CONDITIONS OF SERVICE
The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.
TYPE OF CONTRACT / Fixed TermGRADE AND SALARY / Str LAT / Clinical Fellow
- If appointed as an StR (Locum Appointment for Training) the salary range will be £29,705 £ 46,708 per annum (pro rata)
- If appointed as a Clinical Fellow the salary range will be £ 29,705 £ 39,300 per annum (pro rata)
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
HOURS OF DUTY / Full Time48.00
SUPERANNUATION / You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension.Employee’s contributions to the NHS Scheme are Tiered based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A Personal Pension is a private arrangement agreed with the pension provider that will be an organisation such as a Bank, Building Society or Insurance Company.
REMOVAL EXPENSES / Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.
EXPENSES OF CANDIDATES FOR APPOINTMENT / Candidates who are requested to attend an interview will be given assistance with appropriate travelling expenses.Re-imbursement shall not normally be made to employees who withdraw their application or refuse an offer of appointment.

DISCLOSURE SCOTLAND / This post is considered to be in the category of “Regulated Work” and therefore requires a Disclosure Scotland Protection of Vulnerable Groups Scheme (PVG) Membership which currently costs £59.00. The cost of the PVG Membership will be initially paid by NHS Greater Glasgow and Clyde and will required to be repaid through a payroll deduction mandate from the successful candidate’s first salary.
