Information Packet Required for Selection
Directions: Please complete all sections. Print all required attachments and submit it to Mrs. Robbins in room 923 by the deadline of Monday, September 22nd. If the form fields are not sufficient, you may attach additional information clearly labeled with the sub-section titles. Do not be modest. Every piece of information will be used by the Faculty Council to assist with the fair consideration of your candidacy during the selection process. Please limit the information given to your high school career only.
Completion and submission of this form does not guarantee selection. In order to be selected, you must demonstrate outstanding achievement in all of the following: scholarship, leadership, service, and character. You may be contacted, after your information packet is submitted, for additional information. If you have any questions regarding the completion of this packet or the selection process, please contact Mrs. Robbins.

In order to be eligible for selection, you must have earned a minimum of a 3.5 cumulative non-weighted GPA in all of your A-G coursework[1] taken on the Granite Bay High School campus. Please calculate your GPA using the below table and subsequent instructions.
In addition to completing this form, you must also attach an Aeries printout with all of your previous grades and overall GPA. (Note: This will not necessarily match the result of your below calculations.)

Additionally, one of your previous GBHS instructors will be randomly selected by the NHS advisor and asked to attest to your character.

VIII. Contribution- Selection for membership in the National Honor Society is a prestigious honor. Please tell us, the Faculty Council and Advisor, in a paragraph no longer than one page, what you believe you will bring and contribute to the GBHS chapter of NHS. Remember that selection is based on scholarship, leadership, service, and character. Please type this paragraph and attach it to this packet.
