AT&T Premium ADSL Single-User Rebate Promotion

Please note that the customer must complete the rebate form and the Covad website at and mail the form to the designated address. Once approved, Covad will be sending a check to the customer for the applicable rebate amount, approximately 8-10 weeks after the DSL service has been turned up.

Description: AT&T and Covad are sponsoring a $200 rebate, on Premium ADSL Up to 3.0/Up to 384 Internet Service.

What: AT&T customers can now take advantage of a $200, Covad sponsored rebate, on AT&T Premium ADSL Up to 3.0/Up to 384 orders, submitted and accepted, between July 1, 2005, and September 30, 2005. To qualify for this promotion, qualified Premium ADSL orders must be provisioned through Covad. Applicable orders placed during this time period are eligible for this offer. For the Online Order system, this means orders accepted by the customer. For GCSMIP, this means the contract has been approved, and you have submitted the order.

When: The promotion begins on July 1, 2005, and ends on September 30, 2005. Rebate forms must be submitted to the address indicated on the rebate form, by October 31, 2005.

Customer Benefits: AT&T customers will receive a $200 rebate check, on qualified Premium ADSL Up to 3.0/Up to 384 orders, provisioned through Covad.

Action Required:

After submitting the order, and AT&T accepting the order, you will need to complete the rebate form at and mail the form to the address indicated, by October 31, 2005. All required fields on the rebate form must be completed.

  • Qualified customers will receive rebate checks directly from Covad.
  • Rebate checks will be mailed only to the address where AT&T DSL service was installed. Covad, AT&T’s supplier, will be mailing the checks approximately 8-10 weeks after the AT&T Premium ADSL Up to 3.0/Up to 384 service has been tested and turned up.
  • There is a limit of one rebate per line order, name, address, or household. Duplicate requests will not be honored. Current Covad customers (either through AT&T or another ISP) are excluded from this offer.
  • Only new AT&T Premium ADSL Up to 3.0/Up to 384 orders provisioned through Covad, are included in this rebate promotion.
