Scholarship Application Form
Deadline: December 1, 2017
· Complete this form, sign it, and return it and official transcript(s) (including SAT and or ACT scores) to the Heyl Academic Advisor at your high school by December 1, 2017.
· Have sent at least one and not more than two science/math teacher evaluations to the Heyl Academic Advisor at your high school by December 1, 2017. You may also use an instructor from post secondary coursework for this requirement.
· Submit your Kalamazoo College Early Action (or Early Decision) or Western Michigan University application for admission by November 1, 2017.
· WMU Bronson School of Nursing applicants, complete and submit the supplemental application.
· Make an appointment with the Executive Director or Dr. Lagerwey for the required interview. All applicants must be interviewed. Interviews will be held between December 7, 2017 and January 13, 2018. It is your responsibility to contact the Executive Director or Dr Lagerwey.
