EMPLOYEE JOB DESCRIPTION

Specific Duties & Responsibilities
1.  Provide supervision of children in and out.
2.  Prepares meals as needed in the absent of housekeeper.
3.  Follows individual assignment posted and assigned duties.
4.  Willingness to learn and adapt to changes.
5.  Fills out Bed checks correctly daily.
6.  Fills outs General Care sheets daily.
7.  Fills out 24 hr Cont. Feed sheets daily
8.  Follows through on activities and participates in activities, directed by therapist and other staff as assigned.
9.  Willingness and ability to provide total care to child.
10.  Follows scheduling, positioning, and utilizing adaptive equipment as per scheduling/kardex.
11.  Identifies and communicates needs of children to supervisor and other team members in appropriate manner.
12.  Attends scheduled in-services.
13.  Accompanies children on field trips as needed.
14.  Maintains open lines of communication with all supervisors/staff.
15.  Charge wheelchairs of each client each night as needed.
16.  Empties all trash cans and placed all garbage bags into dumpster.
17.  Clean assigned wheelchairs weekly and document on W/C form.
18.  Document/report when items are broken in group home on clipboard and to Program Manager.
19.  Wash, rinses, sanitizes dishes and put away.
20.  Clean kitchen tops, sweep & mop under cabinets.
21.  Places dirty clothes and linen in washer, dry and fold all clothing and linen and put away as assigned.
22.  Replace/return personal care items into kit after care is given.
23.  Implement and/or supports behavioral interventions as recommended.
24.  Follows and implements Infection Control. Policy and procedures as assigned.
25.  Cleans lounge refrigerator in lounge as assigned on assignment.
26.  Shopping as needed.
27.  Label all open containers before putting in refrigerator with name, date and time can was opened.
28.  Reads Kardex daily. Following through with position schedule. AFO and splint storage and scheduling.
29.  Read kardex for feeding schedule and Daily Living Skills.
30.  Document in transportation log whenever staff accompanies a child to a field trip/schools.
31.  Maintain documentation in mileage log.(Drivers)
32.  Maintain documentation in vehicle inspection log.(Drivers)
33.  Report to Program Manager/Supervisor when there are problems with vans. (Drivers)
34.  Take temperatures as assigned (as needed).
35.  Works as a TEAMPLAYER with other co-workers.

Specific Duties & Responsibilities
Other Duties:
1.  Remove all items from school bags in afternoons. Give any notes from school/teacher to Director or Charge Nurse of any immediate notes that must be addressed from school staff.
2.  Label all feeding bags.
3.  Prepares bags for outings with formula, extra clothing, etc
4.  Weights children once a month as per scheduled with Charge Nurse
5.  Organize client’s closets and dresser draws daily as assigned on assignment.
6.  Stock formula in kitchen when left in the kitchen, as needed.

CHILD CARE WORKER

(POSITION)

1. Certified Nurse Aide (12 week vocational course=165 hrs) or Home Health Aide (215 hrs) preferred High school diploma/GED not required

2. High School Diploma or GED required if not HHA or CNA

3. Valid Florida driver’s license, 3 years driving experience and an acceptable

MVR (not required for classroom staff or midnight staff; other positions

require waiver by Director.

4. Ability to bend at the waist, lift 50lbs and hold child on lap.

5. Ability to read and write English

6. Meets DCF background clearance criteria.
