8. / BRIEF RESUME OF THE INTENDED WORK
Introduction
The human heart is a muscular organ that provides continuous blood circulation through the cardiac cycle throughout the body and is one of the most vital organs in the humand body. it is made up of a collection of tissues. the heart has a mass of between 250 and 350 grams and is about the size of a fist.1
Cardio vascular diseases are becoming a leading cause of morbidity and mortality in industrialized countries and they are also emerging as prominent national health problem in developing countries. As per World Health Organization cardiac diseases are the 1st leading cause of death in developing and under developed countries (9.4%), immediately after lower respiratory tract infection. the picture is worser in developed countries, where cardiac problems cause 13.4% of total deaths. on a whole, total deaths due to cardiac diseases worldwide are 7.20 million (12.2%).2
The projections of increased mortality and increased life expectancy suggest that by the year 2020, cardiovascular diseases will be the principle cause of mortality throughout the world.
6.1. NEED FOR THE STUDY
Due to the advancement in the technology and medicine, we are now able to fight with cardio vascular diseases more effectively and efficiently. the artificial cardiac pacemaker is one of the sophisticated electronic instruments, which are used to initiate the heart beat when the S A node is seriously damaged and is unable to act as the pacemaker of the heart.3
Pacemakers are of two types. one is external pacemaker and the other is internal pacemaker. External pacemakers are always temporary pacemakers and internal pacemakers are permanent pacemakers.4
The common indications of pacemaker therapy are of chronic atrial fibrillation, fibrosis or sclerotic changes of cardiac conduction system, hypertensive carotid sinus syndrome, sinus syndrome, sinus node dysfunction, tachyarrhythmias and third degree AV block.5
A study was conducted to evaluate the effectiveness of planned teaching programme on cardiac pacing among third year B.Sc nursing students of selected colleges of nursing. randomized sampling technique was used to select fifty students. pre testing was done with specially formulated knowledge assessment questionnaire. On the same day planned teaching programme was given. The post test was conducted after seven days and the result indicated that there is significant gain in post test knowledge score (t=8.4, p < 0.05)6
During the investigators service as a staff nurse in an established cardiac super speciality hospital, investigator had met a number of final year nursing students without any knowledge about pacemaker or the care of patient with pacemaker and also about the complications of pacemaker implantation. Very often, physicians leave their patients under nurses care and it will be very tough for nurses to handle the cases without adequate knowledge.
So the investigator felt, a teaching programme for final year students regarding pacemaker can refresh what they have learned in the second year, which will help them to provide better service in the cardiac ward and ICUs, in the immediate future. Discussions with the experts and review of the related literature also had motivated the investigator to undertake the following study.

6.2. REVIEW OF LITERATURE
A describe study was conducted to assess the knowledge of staff nurses regarding cardiac pacemaker 300 samples were randomly selected and a knowledge questionnaire was administered. the study result showed that mean knowledge score of the nurses was only 21 percentage of questions.7
A study was conducted, on the effect of group teaching programme in a private hospital among staff nurses about management of cardiac pacemaker. 180 samples were selected by purposive sampling technique. The findings suggested that there was significant improvement in knowledge level of nurses about management of pacemaker (t0 = 7.84 p < 0.05).8
A study was conducted to determine the effectiveness of a structured teaching programme on pacemaker implantation among student nurses. Samples were selected randomly and they were 100 in number. A structured knowledge questionnaire was used to assess the knowledge of participants. The study revealed that post test knowledge score (22.9) the students were significantly higher than the pretest knowledge score (10.12)9
A study was conducted to evaluate the effectiveness of planned teaching programme on emergency cardiac management. Purposive sampling technique was used for selection of participants (n=50). This study revealed that mean percentage of post test knowledge score (32.7) was significantly higher than mean pretest score (11.2). This indicated planned teaching programme was very effective to enhance knowledge of students regarding emergency cardiac management.10
A group pretest post test pre experimental study was conducted to evaluate the effectiveness of planned teaching programme on pacemaker implantation among staff nurses of a selected super speciality cardiac hospital. The samples were selected randomly. A structured questionnaire with 32 items used to assess the knowledge. The data’s were analysed by descriptive and inferential statistics. the outcome of the study proved marked improvement of knowledge level of staff nurses about pacemaker implantation.
A one group pre-test, post test, pre experimental study was conducted to evaluate the effectiveness of planned teaching programme on nursing management of a patient with pacemaker among III year and G.N.M. students. Forty students were selected randomly. the study findings revealed that mean post test knowledge score was significantly higher than the mean pre-test knowledge score (t=30.52, P<0.001) showing an effectiveness of planned teaching programme in improving the knowledge of students regarding nursing management of patient with pacemaker.12
6.3. STATEMENT OF THE PROBLEM
“A study to assess the effectiveness of planned teaching programme regarding selected aspects of Pacemaker implantation among IV th year B.Sc nursing students of a selected Nursing College at Bangalore”.
6.4 OBJECTIVE OF THE STUDY
The objectives of the study are
- To determine the knowledge of students regarding selected aspects of pacemaker implantation by using a structured questionnaire.
- To evaluate effectiveness of planned teaching programme regarding selected aspects of pacemaker implantation among IV year B.Sc nursing students
- To find association between mean pretest knowledge score regarding selected aspects of pacemaker implantation among IVth year B.Sc nursing students and selected demographic variables.
6.5 OPERATIONAL DEFINITION
Effectiveness:- In the present study, effectiveness refers to impact of planned teaching program, which is evident by gain in knowledge score regarding selected aspects of pacemaker implantation among IVth year B.Sc nursing students .
Planned teaching programme: In the present study, planned teaching programme refers to systematically developed teaching programme to improve the knowledge of IVth year B.Sc Nursing students regarding selected aspects of pacemaker implantation with the help of selected Audio Visual AID, i.e. L.C.D.
Selected aspects of pacemaker implantation: In the present study, selected aspect of pacemaker implantation refers to concept of pacemaker, problems encountered in cardiac pacing and management of a patient with pacemaker.
IVth Year B.Sc nursing students: In the present study, IVth Year B.Sc nursing students refers to those students of the selected college of nursing and they are studying in the fourth year of B.Sc Nursing course.
6.6.ASSUMPTION
1. Students will have some knowledge regarding pacemaker implantation.
2. Planned teaching programme is an approved strategy of enhancing knowledge.
6.7 HYPOTHESIS
The hypothesis will be tested at 0.05 level of significance.
H1 = The mean post test knowledge score regarding selected aspects of pacemaker implantation among IVth year B.Sc nursing students of a selected nursing college at Bangalore. Will be significantly higher than mean pretest knowledge score.
H2 = There will be significant between mean pretest knowledge score and selected demographic variables.
6.8 DELIMITATIONS
The study will be delimited to IVth year B.Sc nursing students of a selected nursing college at Bangalore.
MATERIAL AND METHODS
7.1 Source of data
Data will be collected from IVth year B.Sc nursing students of a selected college of nursing at Bangalore.
7.1.1. Research Design

7.1.2. Setting
The study will be conducted at selected college of nursing at BangaloreJ where GNM, B.Sc (N), P.C.BSc (N) and M.Sc (N) courses are conducted and totally more than 400 students are studying
7.1.3. Population
IVth year B.Sc nursing students, among selected nursing college at Bangalore comprises population of the study.
7.2 METHOD OF DATA COLLECTION:
7.2.1.Sampling Procedure
Purposive sampling techniques will be used to select the samples.
7.2.2.Sample Size-
Total number of students selected for the study will be 50.
7.2.3.Inclusion Criteria
- Students who are willing to participate in the study.
- Students who can be communicate in English.
7.2.4.Exclusion criteria
- Students who are not willing to participate in the study.
7.2.5. Instruments intended to be used
The instruments intended to be used for the study are the following

Implantation
7.2.6. Data Collection Method
Prior permission will be obtained from the /higher authorities of the institution. The purpose for the study will be explained to the students and confidentiality will be assured informed written consent will be obtained. A structured questionnaire regarding selected aspects of pacemaker implantation will be distributed to the participants and requested to be filled. The filled data will be collected back and on the same day planned teaching programme will be conducted for 45 minutes with the help of L.C.D, Post test will be conducted seven days, with the same structured questionnaire.
7.2.7. Plan for data analysis
The data will be analyzed by using both descriptive (mean, median, mean percentage and standard deviation) and inferential statistics (chi square and paired t test).

Yes, in the present study, the investigator plans to fuse structured knowledge questionnaire to evaluate the effectiveness of planned teaching programme regarding selected aspects of pacemaker implantation among fourth year B.Sc (N) students.
