INTERNAL MEDICINE RESIDENCY PROGRAM

CURRICULUM

Harbor Gastroenterology Consult Service

A.  ROTATION PURPOSE: The purpose of this rotation is to provide clinical experience with patients having gastrointestinal and liver diseases in the inpatient and outpatient setting.

B. The CLINICAL EXPERIENCE may include patients having:

1. Reflux Esophagitis

2. Esophageal Motility Disorders

3. Esophageal and Gastric Cancer

4. Peptic Ulcer Disease

5. Inflammatory Bowel Disease

6. Motility Disorders of the Small and Large Intestine

7. Pancreatic Disorders

8. Infections and Manifestation of AIDS in the GI tract

9. Colon Carcinoma

10.  Diverticular Disease of the Colon

11.  Disorders of the Biliary Tract including Cholecystitis and Cholelithiasis

12.  Liver Diseases including Acute and Chronic Hepatitis, Liver Tumors, Wilson’s Disease, autoimmune hepatitis and hemochromatosis

Reading, discussion, and other educational activities should be directed at understanding the pathophysiology, clinical course, and management of these disorders.

C. CLINICAL SKILLS to be developed on this rotation include:

1. focused history and physical examination directed at gastrointestinal liver diseases

2. management of patient with upper an lower gastrointestinal bleeding

3. evaluation of patients with dysphasia and heartburn

4. evaluation of patients with abdominal pain

5. evaluation of patients with jaundice and elevated liver enzymes

6. knowledge of gastrointestinal cancer diagnosis, staging and treatment

7. knowledge of liver cancer diagnosis staging and treatment

8. evaluation of patients with hepatoma

9. evaluation of patients with biliary disease

10. performance of flexible sigmoidoscopies (elective)

D. Exposure to these PROCEDURES, including indications, contraindications, and complications, is likely on this rotation:

1. flexible sigmoidoscopy

2. colonoscopy

3. upper endoscopy

4. paracentesis

5. liver biopsy

6. ERCP

7. esophageal and anorectal motility studies

8. 24 hour esophageal pH measurements

9. endoscopic ultrasound

E. Things to Do As the GI Resident at Harbor UCLA

1.  Refer to St. Mary Survival Guide for Starting at Harbor-UCLA page 36.

2.  First day of rotation: Go to the PCDC clinic Module A at 8AM (if starting on Mon), otherwise meet in the GI lab on the 2nd floor. GI fellow pager: 310-501-3698.

3.  Consults are done in the morning and rounding is done after noon conference.

4.  The Resident must make a calendar of his or her schedule that includes days of ICU call, continuity clinic days, days off and hand it to the Fellow.

6.  On call in the ICU 2-4/month

7.  Attend ½ day of continuity clinic each week. See amion.com
