If selected to be a member of the Metcalfe County High School National Honor Society, you are expected to:
- Maintain a 3.5 or higher GPA
- Successfully complete 3or more higher level classes before graduation
- Attend the monthly meetings
- Complete the required service hours both individual (20 hours) and group (20 hours)
- Be responsible for maintaining the standards that are addressed in the charter by-laws

Please complete all sections. The Application Form maybe typed or printed neatly. Do not be modest as every bit of information can be used by the NHS Faculty Council to assist with
the selection process. Completion of this form does not guarantee selection. If additional space is needed, please attach separate pages. Do not write on the back of these sheets. Applications past the deadline(August 30, 2013) will not be accepted.

Explain why you would be a worthy candidate for NHS. Include examples of your Scholarship, Leadership, Service and Character.
