Dear Parents,
Your son/daughter is interested in becoming a member of the Cheerleading Team for the 2015-2016 school year. If he/she is elected to be a cheerleader, there are specific responsibilities and obligations which your son/daughter must assume in order to qualify for and to remain an active member of the team.
After reading the attached constitution and fully understanding the rules and regulations which govern this program, and after your child has a complete understanding of his/her responsibilities, please sign and have your child sign, as well as return, to the principal’s office by May 8, 2015.
Being a member of the Mount Tabor Cheerleading Team has many rewards and is a valuable experience they will remember for a lifetime. Additionally, your son/daughter will give pride and spirit to our school, students and community through their participation! We are delighted to offer your son/daughter the opportunity to join one of the finest organizations at our school and we look forward to a successful and great year!

2015 Cheerleading Constitution
Mount Tabor High School
I. ORGANIZATION
A. This Constitution has been written and adopted for the Mount Tabor High School Cheerleading Team. The team and its Constitution are governed by Mount Tabor High School and the Cheerleading team members.
B. Henceforth, the name of the organization shall be Mount Tabor High School Cheerleading Team.
C. The purpose of the Mount Tabor High School Cheerleading Team is to:
- Create school spirit, pride and loyalty.
- Promote interest in school activities and perform at school games.
- Develop responsibility, teach self-respect, encourage honest effort, strive for perfection and develop character.
- Teach teamwork and pride in a quality performance through maintaining high standards.
D. Coach Chelsea Beamer was hired by Winston Salem/ Forsyth County Schools upon recommendation of the Principal.
- The Coach will be responsible for the daily running and activities of the Mount Tabor High School Cheerleading Team.
- The Coach will be responsible for administering all disciplinary procedures.
- The Coach, with the team leaders’ approval, may place certain restrictions on team members.
II. ELIGIBILITY
A. Each member must maintain an overall2.0 (C) grade average.
B. Each member must submit a completed Application Form and Parental Consent/Medical Release Form BEFORE the annual tryouts take place.
- The form must be signed by the candidate.
- The form must be signed by the parent/guardian.
C. There will be no more than 2 Team Leaders/Captains whom will be decided by the coaching staff.
III. ATTENDANCE
A. All students who participate in school activities must be in regular full-day attendance the day the activity is scheduled.
- Full-day attendance is from the beginning of the first hour and remaining until the close of the last hour of classes.
B. To participate in a practice, a student must attend school at least a half day.
- This must be fully documented with the Attendance Secretary.
C. If a member is ill and misses ANY activity, he/she must be at HOME or seeking medical appointments.
D. If a member misses ANY practice, he/she will automatically be replaced by an Alternate for that particular performance.
- An absence from a practice or game must be documented by the parent in writing with the coach.
- All doctor appointments, etc. should try to be scheduled so they do not interfere with team activities.
E. All members must attend each activity in its entirety.
F. Membership
- Selection to last year’s team does not guarantee future selection to the team. You are required to tryout every year. Your actions in the presiding year play part in future selection of the team.
IV. UNIFORM
A. Each member will be required to maintain good condition and cleanliness of his/her uniform on a weekly basis.
B. Each member will be required to have all uniforms cleaned and in good condition before the end of the year.
C. Footwear and accessories such as makeup, bloomers, ribbons, etc. will be purchased by each member.
D. Any financial problems/concerns should be taken up with the coach.
E. Each member will be expected to act appropriately while in uniform. Acting the following ways in uniform will result in demerits:
- Inappropriate dancing (i.e. grinding, twerking)
- Foul Language
- Making out and/or hanging all over boys/boyfriend
- Fighting with another teammate/student and/or faculty (verbal or physical)
V. MEDICAL
A. Each member must have a complete physical exam before the first scheduled performance.
B. A member must also have a completed medical emergency form on file with the Coach.
- The Coach must have these forms in her possession at all times.
VI. CHARACTER
A. Team members should be leaders within the school and set a good example at all times.
B. Team members should be above reproach maintaining good personal appearance and habits that cannot be criticized.
- Smoking, drinking and/or drug use are grounds for suspension or dismissal by the Coach.
- Each member must abide by those standards set by High School which are found in the school rules.
C. A member must be courteous and friendly to all other team members as well as the student body.
D. A member must be courteous and friendly to other teams and visitors.
E. A member must be respectful to the Coach and a credit to the school.
VII. POINT/DEMERIT SYSTEM
A. Points or demerits will be used as a disciplinary tool.
B. They can be given by the Coach at the suggestion of the Team Leader/Captain and/or others depending on the situation.
C. Each individual point will result in 40 pushups and 4 laps.
D. Three points will result in the removal from a performance.
E. Six points will result in suspension for two weeks.
F. Twelve points will result in dismissal from the team.
G. One point will be given for:
- Tardiness at any team activity.
- Chewing gum in practice.
- Chewing gum in uniform.
- Uniform inspection.
- Disrespect toward Coach or Team Leaders/Captains and other inappropriate behaviors.
- Inappropriate practice uniform/attire.
- This would include any piece of the uniform.
- This would also include inappropriate hair – It needs to be put up BEFORE practice/games.
- Forgotten props or accessories.
- Inappropriate performance makeup/attire.
- Absence - which will also result in removal from performance.
- Wearing jewelry during practice and/or games.
- Not being in formation by the assigned time.
- Not checking into games on time
- Talking during warm ups.
- Arguing over receiving demerit.
H. Three points will be given for:
- Unexcused absence from practice and/or games.
- Use of inappropriate language in uniform.
- Gossiping in uniform.
- Uniform Violations as mentioned under uniform section.
- Not participating in fundraisers.
I. Six points, or suspension, will be issued for violation of above infractions totaling six points for:
- Smoking in uniform (some schools consider possession on school grounds, or at any activity, grounds for suspension, check your administration).
- Lack of minimal grade requirements.
J. The following violations will result in automatic dismissal:
- Drinking in and/or out of uniform.
- Drug use in and/or out of uniform.
K. Suspension means not attending any team function for two weeks.
- Two suspensions, or twelve points, will result in dismissal from the team.
L. Any school detention or school imposed discipline will be frowned upon by the team.
- School detention, etc. will not be considered an excuse for missing any team practice or scheduled activity. Three points will be issued for such violation.
M. The coach and principal have the right to assign any number demerits for offences not listed on this form that may arise within the school year.
VIII. TEAM LEADERS/CAPTAINS
The roles and responsibilities of Team Leaders/Captains are as follows:
Team captains will follow under the instruction of the coaches. They will help to decide what performances will be performed at the games. They will be responsible of knowing all cheers and choreography and well as calling cheers during games. They will be responsible for helping to make sure their teammates are following rules during practices and games as well as making sure they represent the team and school with a good image outside of practice and games.
IX. BOOSTER CLUB
A. When students become members of the team, their parents automatically become members of the Booster Club.
B. Dates and events are TBD.
C. Parents are required to participate in boosters.
X. CAMP
A. All members are required to attend summer camp.
B. Camp will be held at from 7/31/2015 to 8/2/2015.
- Transportation will be provided for campers.
- Cost will be $264.00, which includes instruction, room and board, and meals.
XI. FUNDRAISING
A. All team members and parents are required to participate in all fundraising which is generated by the team and/or clubs.
B. There will be set goals for each member to attain in their fund raising activities.

- These prices are subject to change. Everything might not be required for every athlete. When the season advances we will have a better idea of what we need.
XIII. TRANSPORTATION
- Each athlete must be able to provide or secure transportation to all home games and practices.
- The School will provide transportation to all away games.
- All athletes are required to go to away games and return as a group unless they ride home with their parents.
- Parents must sign their athlete out when leaving an away game.
Additions to the Constitution may be made at the discretion of the Coach or Principal at any time. Written notification will be provided for any changes.

I further understand that this is an extracurricular activity and that attendance at all practices, games, special functions and summer camp is a requirement of the elected cheerleader.
I hereby give consent to my son/daughter, ______, to try out for cheerleading at Mount Tabor High School and recognize his/her responsibilities and requirements as a leader of his/her school. I understand that, if chosen, my son/daughter will be required to pay for cheercamp, shoes, uniforms, and other required clothing. Further, I understand that personal insurance must cover my son/daughter. I give permission for my child to receive medical attention in the event that I cannot be present or reached for any reason.

I will use modesty when posting photos and/or text to sites/apps including, but not limited to, Facebook, Instagram, Twitter, etc. I understand that if I post anything inappropriate, provocative, and/or derogatory in any way then appropriate action can and will be taken by the coach and/or administration. I also understand that if there are any pictures posted (including pictures you’re tagged in) on sites including, but not limited to, Facebook, Instagram, Twitter, etc. of me drinking and/or using drugs then it will result in immediate removal from the Mount Tabor Cheerleading Team.
