New Office Policy for Dr. Al-Shaikh

Pain Medication Refills

  1. Dr. Al-Shaikh will only refill pain medications for patients that have regularly scheduled appointments for medical conditions. You must be under the care of a physician to be prescribed medications.
  2. All patients must have been seen within the past 3 weeks to have your refill authorized. If you are requesting a refill then you must make an appointment to get a prescription.
  3. Within 2-3 months after surgery no refills for narcotic pain medications will be authorized and patients still having pain will be referred to a pain specialist.
  4. Patients with chronic pain issues will also be referred to a pain specialist.

A pain specialist will be able to offer more treatment options and help with tolerance to pain medications, which in turn will provide you with better pain relief.

  1. Please allow 48 hours for prescriptions to be refilled. No prescriptions will be refilledafter hours or on weekends. DO NOT call Dr. Al-Shaikh on the weekend or after hours for refills- they will not be authorized. If you have an emergency or are in desperate need of medications and it is after hours, please go to the emergency room.
  2. *****Please do not page the doctor after hours or on weekends unless it is urgent. Our office now incurs a significant expense for phone calls after hours and this expense may be passed on to the caller.

United Healthcare

Please Note: We do ACCEPT United Healthcare insurance but we are considered an OUT of network provider. You must have out of network benefits to see an out of network provider. IF you have questions please call united healthcare to verify your benefits.

***It is always the responsibility of the patient, regardless of which health insurance, to verify if the physician is contracted. Please contact your insurance company to verify coverage, deductibles and copays.

Forms/Medical Records

  1. There will be a $10 charge for all forms and please allow 72 business hours for these to be filled out.
  2. If you need medical records copied there will be a $15 charge and please allow 72 business hours.
  3. If your copay is not collected at the time of service there will be an extra charge of $25 for a billing fee.
  4. There will also be a $25 charge for missed appointments. Please allow 24 hours for cancellation.
