Use this form for making changes to the schedule of studies (calendar description) of an associate diploma program.
FORM A: PROGRAM INFORMATION is required when submitting changes todiplomaprogram rules and regulations or changes to admission requirements.

If proposed changes to a Schedule of Studies includesthe addition, deletion, or changes to the sequencing of courses from other campuses, evidence of consultation is required. Submit email correspondence or memo electronically with this form to the Provincial Diploma Program Committee (PDCC) Secretary.

Instructions for using Form D: Specialization and Program Change Template

Current Calendar Description:

1)Copy the most current schedule of studies into a blank MS Word or Word Perfect file and edit to 10 pt font (allows all text to fit in column). Copy this into the “Current Calendar Description” column. To ensure accuracy, use the 2012-2013 calendar copy, NOT the 2011-12 copy. New copy appears online in February 2012 and in draft format in December 2011. If you are unsure whether changes were made to the specialization during the previous cycle, contact the PDCC Secretary or OAC Associate Dean’s Office for the most accurate calendar copy.

2)If you are making changes to a small number of sections (one or two semesters, preamble only, etc.) copy only those sections.

Proposed Revisions:

  1. Using the previously copied calendar description, bold any text to be added (courses, instructions, changes to preamble, notes at end) to be added. Use the strikethrough formatting for text to be deleted. From MS Word or Word Perfect, copy the revision into the box below.
  2. If the format above is not followed, it will be returned to the Regional Campus or OAC Dean’s Office without review.

Submitting Revisions:

  1. Changes affecting multiple campuses must have the approval of each Director. Add multiple signature lines as required.
  2. Provided the Director confirms approval in a memo format, the signature may be typed in and the memo included with the submission.
