Create a new “UDS 3 Forms and documentation” web page
Centers can now download the final UDS 3 form packets — initial visit, follow-up visit, and telephone visit — which have been updated to incorporate a number of the thoughtful suggestions received from Centers during the review period.Links can be found below.
A number of changes were made to streamline the UDS evaluation. For example:
- The Subject Health History Formwill be submitted at the initial visit only.
- The HIS/CVD and UPDRS forms have been eliminated (with important elements from these forms moved to Forms B8 and D1).
- For follow-up visit Form B9 (Clinician Judgment of Symptoms), a code for “Assessed at a previous visit” has been provided for questions on first predominant symptom and age at onset.
Please try the forms on at least a few subjects and let NACC know by December 1if you discover any problems (please email ).During this testing period, Centers should NOT submit the resultingUDS 3 data to NACC (no UDS 3 data should be submitted to NACC until after the UDS 3 implementation date, at which time UDS 3 data should be submitted only for visits conducted on or after the March 1, 2015 implementation date).Any changes that come out of Center testing will be incorporated into the forms, and updated forms will be distributed to Centers during the first week of December.
Beginning March 1, 2015(the implementation date), the UDS 3 forms will be required for use with all eligible subjects seen at the Center.Centers will have the option of using NACC’s web data-entry forms, which will be helpful for those that are still preparing their databases to accept UDS 3 data on March 1. Visits completed before March 1, 2015, should be administered and submitted to NACC using UDS 2 forms. NACC will continue to accept UDS 2 forms that were completed before March 1 but that were not submitted to NACC until after March 1.
And finally, as mentioned at the meeting in Baltimore, the ADC Clinical Task Force will be disbanded after December 1; however, CTF members will still be available on an ad-hoc basis if there are any lingering UDS 3 questions that need to be addressed.

UDS implementation time line
October 20, 2014: Centers receive the final UDS 3 form packets (initial visit, follow-up visit, and telephone follow-up).
October 20, 2014 –December 1, 2014: Centers will test the UDS 3 forms on a few subjects and informNACC () of any problems, while not submitting any of the UDS 3 testing data to NACC.
In November 2014: NACC will provide the DED, Data Template, Milestones Form, Neuropsychological Battery Instructions, and Coding Guidebook, so that Centers can begin programming their databases to accept UDS 3 data.
First week of December 2014: Any changes that need to be made after Center testing will be incorporated into the forms, DED, Data Template, and Coding Guidebook, and the updated versions of these documents will be distributed to Centers.All of the changes that have been made since the October 20 version of forms will be listed in detail to help Centers incorporate the changes into their databases.
Now through February 28, 2015:Submit ONLY UDS 2 forms for all subject visits to NACC. (Data collected during this period using UDS 2 forms may be submitted even after March 1, as a delay between data collection and data submission to NACC is expected).
March 1, 2015(the implementation date): Centers must use UDS 3 forms on all subjects with visits on or after March 1.NOTE: At the Center’s discretion, the UDS version 2 neuropsychological battery may continue to be administered for follow-up visits only, and submitted using Form C1; for all new subjects, the version 3 battery must be administered, and submitted using Form C2.
