APPLICATION

Your application must be typed and include all of the following items by the deadline to be considered complete:

I.  Completed and typed application form.

II.  A current copy of the Student Aid Report (SAR) obtained by filing the Free Application for Federal Student Aid. (FAFSA) This contains the Expected Family Contribution (EFC) which will help determine financial need.

III.  For Academic Evaluation, the completed application must include the following documents:

A.  For new applicants who are undergraduates:

1.  A copy of an official Alamo Heights High School transcript.

2.  A copy of ACT or SAT score and date of graduation from AHHS.

3.  A copy of your college transcript showing a completed freshman year (at least 30 hours) with a minimum average of C overall.

4.  If you have not completed 30 hours but will by June of this year, send a current transcript now showing completed work and another by June 15 showing 30 hours.

B.  For new applicants who are graduate or doctoral students:

1.  A copy of an official Alamo Heights High School transcript.

2.  A copy of ACT or SAT score and date of graduation from AHHS.

3.  A complete copy of official college transcripts.

4.  A copy of scores from tests taken by you to enter graduate school (GRE, LSAT, GMAT, etc.)

C.  Current Campbell Scholarship recipients:

1.  A. complete copy of official college transcripts up to and including grades from Fall 2017. (Disregard if Fall 2017 grades have been sent to this office.)

IV.  The following letters mailed (by the deadline) to: The W. W. & Anna May Campbell Scholarship Fund, c/o Alamo Heights School Foundation, 7101 Broadway, San Antonio, Texas 78209.

A.  A letter of recommendation from one of your Professors or your Academic Advisor. (ALL)

B.  A letter of recommendation from one adult who is not a relative (do not need this from current recipients).

C.  In addition, please include a short personal letter outlining any additional personal, financial or academic information which you feel will help us to evaluate your application fairly.

The purpose of the W. W. & Anna May Campbell Scholarship Fund of the Alamo Heights School Foundation is that it be an endowment fund whose income is to be given in the form of scholarships to graduates of Alamo Heights High School who will attend a public college or university and who have completed the equivalent of at least one year (thirty hours credit). Such qualities as leadership, community involvement, work history and special professional qualifications will be considered, along with the applicant’s degree of financial need and academic excellence or diligence.

The attached form is to be completed by all applicants for scholarship aid. All information used in evaluating the application will be inaccessible except to members of the Scholarship Committee, who will hold it in the strictest confidence.

In their efforts to ensure that the donor’s wishes are carried out as completely and responsibly as possible, the Scholarship Committee, which is to review all applications, asks that all applicants and their families sign below:

I (we) certify and acknowledge that the Scholarship Committee of the Alamo Heights School Foundation shall have the authority and permission to obtain any information it deems necessary from administrative officials at schools I have attended or plan to attend; that the Committee’s recommendations will be submitted to the Board of the Alamo Heights School Foundation, which has the sole authority to award funds; that any irregularities found in this application, or change in circumstances occurring after submission of this application (such as change of school, failure to maintain grades, etc.) may result in the re-evaluation, revocation, and/or withdrawal of any scholarship previously awarded, at the sole discretion of the Scholarship Committee; that the statements made in this application are true and correct to the best of my (our) knowledge and belief; that in the event of a change in circumstances after the date of submission of the application, I (we) agree to submit a supplement to the application containing information on the change in circumstances.
