Please fill out the request below and mail to the Bursar’s Office at Campus Box 14, Greeley CO 80639 or fax to 970-351-1142. Requests must be sent in by December 15th of each year to ensure that you will receive a paper 1098T.

THE STUDENT MUST REQUEST THIS INFORMATION.
