RELIEF SUPPORT WORKER

Job Title: Take Home and Settle Worker (Evening East Sussex)

Purpose of post: To collect patients (clients) from the Accident and Emergency department and wards at Conquest Hospital (Hastings) and Eastbourne District General Hospital. To settle them at home and assist in the prevention of readmissions into hospital through the early identification of health and social care needs and potential hazards in the home.

Accountable to: Take Home and Settle Team Leader and in her absence the Health & Community Development Manager

Location: on call from home covering East Sussex

Hours: To provide cover as and when required between 6pm – 10pm Monday – Sunday. It is also expected that you will be on a back up rota to cover the Service in the event of a support worker’s sudden absence for which you will be remunerated.

DUTIES AND RESPONSIBILITIES

1.  To collect clients from hospital within an hour of receiving the call out.

2.  To drive clients to their home and settle them in.

3.  To undertake a home risk assessment and identify any risks to the client

4.  To undertake a person centred assessment to highlight areas of need identified by the client and/or self and to act according to the Service procedures.

5.  To act at all times in accordance with the Organisation’s policies and procedures.

6.  To have a direct line of communication to the Take Home and Settle Team Leader or in her absence the Health & Community Development Manager for supervision and support.

7.  To maintain accurate records on clients receiving the Service.

8.  To maintain accurate records which monitor the performance of the Service.

9.  To ensure that core skills and knowledge are updated including updates on Age UK East Sussex’s policies and procedures.

10. To maintain good lines of communication with the Hospitals.

11. To promote the service with hospital staff.

12. To record monthly mileage and time sheets.

13. To attend meetings when required.

14. Other tasks as required by the Health & Community Development Manager.

15. To understand and act within our quality frameworks (currently ISO 9001 and Age UK Quality Standards).
