Know all men by these presents that Good Shepherd Episcopal Church of Friendswood, Texas, in consideration of the payment of the reservation fee of ______Dollars ($_____.00), payable:

a.)  in full upon execution of Applicant’s Contract of Inurnment Rights and this Certificate of Inurnment Rights, or

b.)  ______Dollars ($______.00) upon execution hereof and the balance on or before that date six (6) months after the date of execution hereof, or

c.)  ______Dollars ($______.00) upon execution hereof and the balance on or before that date one (1) year after the date of execution hereof;

receipt of which is hereby acknowledged , hereby grants to ______, whose address is: ______as Right Holder, for inurnment purposes only and subject to the conditions, reservations and restrictions herein contained (and as further stated in the contract of inurnment rights and the rules and regulations pertaining to the columbarium), the limited right of use of the following space situated in the Columbarium of Good Shepherd Episcopal Church of Friendswood Texas:
