PARENTAL CONSENT FOR MINORS – Any player under the age of 18 on the date of registration must have a parent/guardian’s signature below and include a photocopy of the player’s birth certificate with this form before registration can be accepted.

Completed forms must be sent to the fixtures secretary. Players that do not have an existing ID card must send 2 x passport size photos.

Player Registration and ID cards 2012/13

The process to register a player for the 2012/13 season is as follows:

·  For new players:

Fill in the registration form, include 2 passport sized photos, birth certificate and parental consent if required .

·  For existing players:

Complete and return the form fill with birth certificate and parental consent if required.

·  Send NO money, you will be invoiced in due course.

·  Please send a SAE for the ID cards to be returned.
