  • Employer of the Year - Multiple winners based on company size.
  • Rookie of the Year - A company just getting started with a great new wellness plan.
  • Best Company/Group Wellness Event - Awarded to the best employee or community event!
  • Health and Wellness Champion of the Year - Someone who embodies healthy living in the workplace.
  • We recommend using this form to help you prepare, but then ask that you submit your nomination onlineat WealthOfHealthAwards.com.
  • Offline submissions will not be accepted.

Wellness Program Objectives and Design

1 / What are the top three goals of your wellness program?
2 / How does your program go beyond the traditional focus of physical health (nutrition/physical activity/biometric screening) to support the overall well-being of your employees?

Wellness Program Support and Engagement

3 / Who coordinates and promotes your wellness program? Please describe who is involved in planning, promoting and evaluating your wellness program. (For example, is employee wellness part of someone’s job description; is there a wellness committee or wellness champions?
