Job Title:Dementia Adviser

Salary:

Hours:35 hours

Accountable to:Care Services Manager

Job Summary

To be responsible for ensuring that people with dementia & their carers, are provided with advice, information, advocacy, emotional and practical support.

Main Duties

  1. To carry out the duties of the post with full regard to Equal Opportunity policies.
  1. To respond to the individual casework needs of people with dementia, and their carers, including emotional and advocacy support.
  1. To be responsible for providing specialist advice and support for people with dementia & their carers
  1. To support people with dementia, & their carers, in accessing services and to identify all resources, which are available or may be required to meet needs.
  1. To undertake home visits and attend meetings as required
  1. To assist people with dementia to resolve problems of an emotional nature often related to their diagnosis and interpersonal relationships, particularly where these factors block the resolution of more practical problems.
  1. To help people with dementia to formulate and implement goals and plans to help them come to terms with their diagnosis and to enable them to remain active members of their community.
  1. To facilitate and support people with dementia to visit their GP to seek additional medical support where appropriate.
  1. Carry out health checks for people with dementia as appropriate
  1. To collaborate closely with other care provision partners in the statutory and voluntary sector to ensure that a multi-disciplinary assessment and holistic response to people with dementia’s needs is made
  1. To increase awareness of the needs of people with dementia amongst statutory and voluntary professionals in Trafford
  1. To organise and facilitate support groups, informal support networks, social activities and information events for people with dementia and their carers as and when required.
  1. To enable people with dementia and their carers to be involved in any appropriate research opportunities
  1. To contribute to the bi-monthly newsletter and our social media activities
  1. To provide written information to support people with dementia.
  1. To maintain accurate and relevant records and prepare written reports utilising information technology systems, as required and adhering to the organisations policies of confidentiality.
  1. To be familiar with appropriate legislation, procedures and resources in order to ensure that the statutory requirements are met.
  1. To participate in strategic planning sessions where appropriate.
  1. To represent the views of people with dementia in any strategic meetings relating to Services in Trafford.
  1. To provide information on the unmet needs of people with dementia.
  1. To attend regular casework supervision sessions and line management meetings as required.
  1. To participate in and contribute to training as appropriate to the experience of the post holder.
  1. To work flexible hours (for which time-in-lieu will be given) where this is necessary to meet the needs of service users or the organsiation.
  1. To undertake other comparable duties as may be required in the role of dementia adviser
  1. To promote the good name of Age UK Trafford at all times.

Hours of Work: 5 days per week 9.00-4.30 (includes ½hr unpaid lunch break)
