Agenda Item #1 Welcome/Introductions and updates:

  • We reviewed our mission statement and purpose for being together, we are there for each other and our families and know that when you have committed people like this collaboration has, the community becomes stronger and a strong community leaves our families feeling supported and better able to nature and care for their children. We are making a difference in the lives of families today and for years to come!

Discussion:

Group sharing:Everyone introduced themselves and shared information that builds community! We had several new people that joined us today and we are so thrilled to have them! Please know you are so welcomed and important to this community!

  • Emily Martin with Big Brothers Big Sister shared they are holding their 23rd Annual Chili/Salsa cook off for a cause on September 17th 11:00- 3 pm at the Hard Rock in Battery Park.
  • Edith Rohde shared that Ameri Health will be at the Convoy of Hope on Sept. 17th.
  • Sharon Skinner with Boys and Girls Home shared they will be having their Tailgating for Kids fund raiseron Friday September 23rd. If you would like to be part of the please contact Sharon.
  • Susan Strahm with Early Learning Connections shared that the Tri State Early Childhood conference will be Oct. 1st and they are expecting around 300 people.
  • Jim Steele our new South Sioux City Chamber President shared that the Chamber golf outing was a great success!
  • Tonya Bauman with Building Blocks shared that September is National Foster Parent month and if you know people who are doing this work be sure to recognize them.
  • Sally Reinert shared the are starting preparations for the Cardinal Christmas Baskets and if you know someone in need be sure to let them know.
  • Judith Stanton encouraged the leaders in our community to work with seniors
  • Corrin Mikkelson form the Y shared they are starting a program called healthy weight and they still have a few openings. The program is for children that are overweight.
  • Jen Ankerstjerne with Dakota County Health shared they are putting together a volunteer group to help in case there was an emergency like an epidemic that broke out. They will be meeting at 7:00 pm on Oct 15th at the court house and all are invited to come.

Agenda Item #2: Dakota County Connections (DCC) Community Goal

  • We will be updating the DCC goal in a couple months, currently we are waiting on updated Nebraska Crime Commission data.

Agenda Item #3: Dakota County Connections & SHIP Chamber Coffee Hour September 16th at the College Center 9:30! Please come and share with others about the great connections we have together!

Agenda Item #4: (RIR) -PYRAMID MODEL

  • The Pyramid Model teaches how to support social and emotional competence in infants and young children. Providers learning everything from how to set up a classroom for best results to learning how to best engage with young children. We have had our kick off meeting and are now implementing Up to date training utilizing all the information in the Pyramid Model for Center staff and In-Home providers with onsite coaches to support learning.
  • PIWI (Parents interacting with Infants): A New Class has started at the Pender Community Hospital on Sept. 6-Nov.1. to support social and emotional competence in infants and toddlers
  • PCIT (Parent Child Interaction Therapy) on going for families and children
  • Parent Education and Training classes to support families

Agenda Item #4: Focus Groups

  • Community Response Meeting: (CR)
  • We are now in the process of hiring a part-time Central Navigator who can help families find needed supports in an effective and timely manner.
  • We will be starting simply with referrals from one school to make sure we are able to serve families well and make sure we haven’t missed any steps in serving our children and families.
  • We currently are looking for 3 Coordinated Care Managers or coaches who can be hands on if the family’s situation requires, they would provide stabilization, help develop a family plan and locate services/supports for that family. We are inviting everyone interested in this part of the Community Response support system to send a quick email to JoAnn Gieselman letting her know of their interest and how this would work for them. We will also be looking for many providers who can supply the needed services.
  • CR is an exciting and very important way to serve families before they are in crisis and it’s possible because we are a connected well working Collaboration that together betters our community.
  • Safe and Healthy Youth:
  • This group has joined with other community members to take a deeper look at the issues, barriers and gaps in our community to determine what our next steps might look like in supporting our community. Through our friends at the United Way, some wonderful connections in the schools, and friends in the area we have begun to set up focus groups with families and HR personnel at Tyson. We will be asking those who live here what Challenges their families face in the community and what services may be lacking to meet those challenges.
  • Parent Education and Engagement:
  • Through identifying what is working for our community and what some of the needs of our parents there are three parent trainings selected for the upcoming year. The group selected the Circle of Security (COS) with training provided by Crittenton Center, STEP (Systematic Training for Effective Parenting) also with the Crittenton Center, and 6 Rooted in Relationships Teaching Pyramid parent module trainings with our Pyramid Coach Janie Snieder (A Better Way Therapy) for the coming year. The group will also be selecting kits for parents that address different behavioral issues that can be checked out at the SSC Library with games to play and books to read as well as handouts and information aroundchallenging behaviors. Different types of incentives will be offer to parents for their attendance at classes that will go along with their lessons, and give them fun things to do as a family.

Agenda Item # 8: Presentation: David Faldmo, MPAS, PA-C

Siouxland Community Health Center is a patient-centered medical home offering a full range of medical care.New patients and walk-ins are always welcome. The hours are Monday & Thursday 8am – 8 PM
Tues, Wed, & Friday 8am – 5PM. David Faldmo, MPAS, PA-C, is the medical director of Siouxland Community Health Center, he shared they are currently leasing the SSC building for three years and then will determine their next steps at the end of those three years, the current building is more than they really need at this time. They offer most of the same services that the Sioux City Clinic offers, if they don’t have it at their clinic for example dental, families can still receive those services at the Sioux City clinic.

David shared they wantensure a quality experience for all who choose the health center for their care. One of the things they have been working on is having clients fill out a survey when they come in to get a snap shot of their lives. SCHC recognizes that what is happening in a person home and personal life make a big difference in their health and they can’t “fix” a patient in one appointment at their center. They are looking for ways to support families so their environments and lives are healthier all around. In addition to this work, the health center continues to engage partnerships in the community to increase accessibility and availability of services. Through outreach service sites, referral relationships, and active participation in community planning, the Siouxland Community Health Center is committed to ensuring that they are meeting the needs of their patients.

There were lots of good questions surrounding the presentation many over transportation in our area. A couple agencies were able to offer some options they had. Jennifer Jackson shared there was a grant for providing a bus and she would get more information on that.

  • Defining common outcomes
  • Aligning individual community needs and resources to meet those outcomes
  • Seeking additional resources to help reach our goals
  • Using evaluation and community data to strengthen our programs

Dakota County Connections does not provide programs, it serves to provide a common table for community partners to gather around and collectively address community needs. We support all the organizations and individuals that are directed toward our common outcomes.
