DO NOT RENUMBER THIS DOCUMENT. IF A SECTION OR EXHIBIT DOES NOT APPLY, REMOVE THE TEXT AND INSERT THE WORD “RESERVED” FOR THAT PARTICULAR SECTION. If there is a choice between several provisions, delete all of those that do not apply.

Sections marked in green must be filled in initially by the requestor. Sections in blue font, must also be addressed by the requestor, sometimes independently, and many times in conjunction with the DMB OAS Buyer (see check list matrix and associated responsibility key). The entire document will be reviewed and approved by the assigned buyer, and DMB OAS management (based on buyer signature authority) before being posted on the website for bidders.

Sections marked in yellow are instructions that must be deleted from the final contract.

The attached Request for Proposal (RFP) format has been prepared for use by State agencies when the services of consultants are required. The format is written in a “fill-in-the blank” style and is hopefully self-explanatory. However, please feel free to direct questions concerning the format to the Office of Acquisitions, Department of Management and Budget, at (517) 373-6327. The RFP format consists of the following four sections:

Section I, Work Statement. This section is a detailed description of the services or study to be contracted for, including the background and objectives of the work. It is the most important portion of the RFP. Generally, the more precise, accurate, and complete this part of the RFP is, the greater will be the probability of receiving proposals that meet agency needs. An essential feature of article I is the segmentation of the work into specific tasks if the nature of the study warrants such a breakdown. Content of this section is the responsibility of the requesting agency.

Section II, Contractual Terms and Conditions. This section describes the contractual terms and conditions of the RFP that will be a part of any Purchase Order (P.O.)/Contract that results from the RFP. Content of this Section is the responsibility of the Office of Acquisition Services, but does include areas requiring agency input.

Section III, Certifications and Representations. This section asks potential Contractor(s) to certify and assure the State of certain requirements regarding their firm and or their ability to perform the work described within the RFP. Content of this Section is the responsibility of the Office of Acquisition Services.

Section IV, Bidding Information. This section is used to inform bidders of the general procurement conditions under which the RFP is issued. Included are informational items concerning a general statement of the services or study to be contracted for, time constraints, rights reserved to the State, response date, preproposal conferences, oral presentations, news releases, and contractual information. Content of this section is a joint responsibility of the Office of Acquisition Services and the requesting agency. Also included within this section is Information Required From Bidders, this section provides bidders with instructions regarding the format and nature of the detailed information that must be provided in a proposal. Content of this section is a joint responsibility of the Office of Acquisition Services and the requesting agency.

These four articles contain language and/or format for the RFP. All of the language used has been carefully chosen for applicability to State business. Much of the language, especially in Section II, has been prepared in cooperation with the Attorney General’s office, and is not to be modified by State agencies without consultation with the Office of Acquisition Services. Should a Blanket Purchase Order (Contract) result from the RFP, its format and language will consist of sections I and II of the RFP and the winning vendor’s proposal. The RFP and all appendices and attachments are to be prepared by the requesting agency in MS Word. Electronic copies of RFPs are to be submitted via e-mail or diskette.

The purpose of this RFP is to obtain a bid for a consultant to provide services to the State of Michigan, Department of Education, Office of Safe Schools (OSS), within the Coordinated School Health and Safety Programs Unit.

In addition to delivering services related to local and state requests for technical assistance, the Office of Safe Schools is responsible for achieving project outcomes related to the Title IV Data Management grant. The consultant will assume the roles of Project Coordinator, and will be responsible for providing these services for the OSS, The consultant will work jointly with the Project Director (already on staff within the Michigan Department of Education) on all duties that fall under this ITB.

The Project Coordinator will manage, coordinate, train, and monitor LEA and ISD efforts to incorporate data driven decision making systems and resources into their school-based drug and violence prevention efforts. The consultant will serve as a liaison between schools and effective programs, practices, and policies. The consultant will link their network of prevention professionals in Michigan with the resources needed to accomplish the goals of the OSS. In coordination with a variety of prevention units in multiple state agencies, the consultant will offer services, which include the following activities:
