PETITION FOR REQUISITION
TO THE GOVERNOR OF MISSOURI:
The undersigned, State of Missouri, County of Christian, Office of the Prosecuting Attorney, by Amy J. Fite, Prosecuting Attorney for the County of Christian, State of Missouri, based on information and belief he has received in his capacity as Prosecuting Attorney, alleges:

1. That Defendant, «DEF_FIRSTNAME» «DEF_MIDDLENAME» «DEF_LASTNAME» «DEF_NAMESUFFIX», stands charged in the Christian County Circuit Court, Associate Division by the accompanying authenticated copies of COMPLAINT FOR STATE WARRANT, WARRANT FOR ARREST AND PROBABLE CAUSE from (Insert Title) «REF_OFFICERFIRSTNAME» «REF_OFFICERLASTNAME» with one count of a class (Insert class) felony of (Insert charge) in violation of the accompanying Section (Insert Statute Number), RSMo.; and«TableEnd_Case»,
2. That on or about the «TableStart_Case»«Ref_OffenseDate»«TableEnd_Case», the said defendant was personally present and committed acts in the County of Christian, State of Missouri, which resulted in the intentional commission in the State of Missouri of the crimes for which he has been charged in the County of Christian, State of Missouri, and the circumstances of the commission of the crimes are alleged as follows: in that on or about the «Ref_OffenseDate», the defendant appropriated credit cards, which property was owned by E.N., and the defendant appropriated such property without the consent of E.N. and with the purpose to deprive her thereof; and
3. That the said defendant is presently in the County of (Insert County), state of (Insert State), after fleeing the state of Missouri, and further alleges: That the ground for such belief are as follows: a message received from the North Star Borough County Sheriff’s Department that the defendant, «TableStart_Case»

WHEREFORE, your petitioner asks that a requisition issue upon the Governor of the State of (Insert State), and that Christian County DEPUTY SHERIFF LEE GRIFFITH may be appointed messenger of the State of Missouri with ONE GUARD, to go after, receive and return to the County of Christian, State of Missouri, the said defendant for trial.
