I give Posh Paws Pet Spa permission to seek emergency medical attention for my pet if it is deemed necessary. I assume all liability and financial responsibility for any and all costs involved for any medical attention sought for my pet's well-being. Grooming: I am aware that if my pet is matted Posh Paws Pet Spa may deem it necessary to "dematt" or even shave my pet. These procedures may have unpleasant consequences such as clipper/brush burn or nicks or cuts. Posh Paws Pet Spa will take all necessary care and precautions during ALL grooming services, but will not be held responsible for any side effects, injuries or medical bills incurred. Boarding: I give permission for Posh Paws Pet Spa to board my pet over night at a charge of $22.50 per day if I do not arrive by closing time to pick up my pet..

Pet Form For Matted Or Severely Tangled Pets

Your pet is important to us. Because we care, we want to assure you that every effort will be made to make your pet’s visit as pleasant as possible.
If your pet is severely tangled or matted, it is at greater risk of injury, stress and trauma. All precautions will be taken. However, problems occasionally arise, during or after grooming, such as nicks, clipper burn and mental or physical stress.
In the best interest of your pet, we request your permission to obtain immediate veterinary treatment should it become necessary. We will try to notify you the owner first, but in the event that we are not able to contact you, we would like your permission to seek veterinary help should the event call for it.
I hereby grant permission to Posh Paws Pet Spa to obtain emergency veterinary treatment for my pet. Also, realizing that Matted Pets have a greater chance of injury during grooming, I will not hold Posh Paws Pet Spa responsible for accident or injury to my pet.

Posh Paws Creative Grooming Release Form
Any and all creative grooming will be done as always in the best interest of your pet. We take great care to make sure the products used for creative grooming at Posh Paws Pet Spa & Resort are safe and no-toxic.
Before any creative groom is attempted Posh Paws must first complete a preliminary groom to ascertain whether or not your pet is a good candidate for creative grooming and what type of creative grooming your pet can or cannot tolerate. These applications may include: semi or permanent color, temporary color, airbrush color, hair chalks, glitters, skin safe non toxic adhesives and bling. As with any topical agent there is always a minimal potential for allergic reaction, if an allergic reaction occurs Posh Paws will not be held liable.
Each individual pet has their own coat type, texture and color. No two grooms are exactly alike, variation in color and how long it may last, may differ from one pet to another. All creative grooms no matter how large or how small are billed by the time it takes to complete them on top of the cost of the groom. We will do our best to estimate what that cost will be. In the best interest of any pet we reserve the right to stop any creative grooming applications if your pet develops a reaction or becomes uncooperative during the process of a creative groom, this does not mean that the design may not be completed it just may need to be completed at a later appointment. Any and all creative grooms will be discussed at length with the owners at the time the appointment is made any changes made to a scheduled design must be stated before the appointment date as I may require time to have supplies needed to complete your pets creative groom.
