  1. What do we want to accomplish (aim)? By May 26, 2016, 6 NHV’s will improve the rate of those that are breastfeeding past 2 weeks in 75% of the moms delivering in March and April and early May.
  1. How will we know a change is an improvement (indicators)?
NHV visits with the mother within 1-2 days after leaving the hospital to specifically address any breastfeeding needs / issues.
Result – providing support through a most crucial time will lead to continued breastfeeding past 2 weeks
  1. What ideas do we have that will result in improvement (change to test)?
NHV has contact with mother preferably by home visit within 1-2 days after returning home from hospital
Questions we want to answer with this PDSA cycle / Will the NHV’s visit provide mom with enough support to help them continue to breastfeeding after leaving the hospital
Will the NHV’s visit help the mother with her questions or fears so that she does not give up on breastfeeding
Will it result in additional breastfeeding support at a crucial time or add more stress
Predictions / 75% of the 9 moms will be reached (by phone or in person) 1-2 days after returning home from the hospital and will be offered support.
50% of these 9 moms will not want a home visit during first 2 days out of hospital but will speak to NHV by phone.
