JOB PURPOSE

Statement of Duties
General
•Coordinate care for eligible patients identified by South Eastern Sydney Local Health District(SESLHD) Connecting Care Program as provided bySESLHD;
•Actively case manage enrolled patients on the SESLHD Connecting Care Program, including recommending appropriate referrals and incorporating information from existing care plans;
•Liaise with the patient's General Practitioner regarding the coordination of care;
•Liaise with the patient’s other health professionals and community service providers regarding the coordination of care
•Attend and participate in the Connecting Care Implementation Committee organised by SESLHD to represent the views of the ESML as a proxy for the ESML CEO as and when required;
•Actively support and educate General Practice staff in the management of chronic disease and coordination of care.
•Create and maintain patient records for the Connecting Care Program including maintaining the confidentiality and security of patient records in line with NSW privacy legislation and the Privacy Statement of ESML.
•Ensure that SESLHD Minimum Data Set information for the program is collected and results areentered correctly into the database provided by SESLHD.
•Manage the relationship and contract with SESLHD ensuring compliance with contract terms and accurate and timely communication and reporting
•Prepare recommendations, reports and proposals as necessary to meet ESMLrequirements and document processes and findings of the program.
•Participate in SESLHD/ESMLcollaborative planning activities and quality assurance activities as they relate to this program.
Other
•Provide timesheets, leave applications and petty cash forms to the relevant ESML staff member for approval.
•Budgets: assist with all financial aspects of program budgets planning and administration
•Contribute where appropriate to Medicare Local's newsletter, website and faxout.
•Participate in relevant training and conferences
•Ensure the confidentiality of any and all information obtained in relation to the position

CONSTRAINTS/AUTHORITYLEVELS
- All purchases must be approved by the COO or Dir CS&PS
- Official correspondence must be signed off by the COO or Dir CS&PSand the signed copy kept on file
- Appointee authorised to determine the need to make business related STD, mobile and tele/video conference calls
RELATIONSHIPS
- ESML Board,CEO,Operations Manager
- Cooperation with all staff, visiting GPs and other persons at all levels.
ESSENTIAL CRITERIA
- Health professional registered with Australian Health Practitioners Regulation Agency (AHPRA)
- Demonstrated high level of both written and oral communication skills
- Sound understanding of the principles of patient self management and health coaching
- Experience engaging with a multidisciplinary team to improve health outcomes for patients
- Ability to work autonomously and as part of a team
- At least intermediate computer literacy and experience in Microsoft Office suite
- Current drivers license
- Good understanding of the Australian healthcare system, especially primary health care
- Experience in program management
- Knowledge of local primary health care services
- Experience in care coordination
- Experience working with CHIME and eMR
