Pursuant to Article 27 paragraph 4, Article 29 paragraph 3, Article 33 paragraph 2 and Article 47 paragraph 4 of the Law on Travel Documents (“Official Journal of Montenegro”, no. 21/08), the Ministry of Interior and Public Administration adopts the
REGULATION
ON THE FORM OF THE REQUEST FOR ISSUING PASSPORTS AND TRAVEL CERTIFICATE, CERTIFICATES OF SUBMITTED REQUESTS FOR ISSUING TRAVEL DOCUMENTS, REPORTS ON MISSING TRAVEL DOCUMENTS AND CONSENTS OF PARENTS OR LEGAL REPRESENTATIVE
Article 1
This Regulation shall stipulate the forms of requests for issuing passports and travel certificates, certificates of submitted requests for issuing travel documents, reports of missing travel documents and consents of parents or legal representatives.
Article 2
The form of the request for issuing passports (Form number 1) shall contain the name of the authority to whom the request for issuing passports is to be submitted, number of the request (bar code), the title: “REQUEST FOR ISSUING PASSPORTS” and boxes for entering data referred to in Article 29, paragraph 1 of the Law on Travel Documents (hereinafter: the Law), and the data on the language and the script used for entering the data on personal name, reasons for issuing passports, telephone number and the place for signature of parents and/or legal representative.
The form referred to in paragraph 1 of this Article shall also contain the following notes: “Citizens must enter correct information in the request” and “The Request is to be filled in readable capital letters”.
Article 3
The form of the request for issuing passports which is to be submitted to a diplomatic or a consular representative office of Montenegro on whose territory citizens of Montenegro reside (Form number 2) shall contain the name and the location of diplomatic or consular representative offices that the request for issuing passports shall be submitted to, the number of the request (bar code), the name: “REQUEST FOR ISSUING PASSPORTS” and places for entering the information referred to in Article 29, paragraph 1 of the Law, and the information on the language and the script used for entering information about the personal name, reasons for issuing passports, telephone number, address of the residence of Montenegrin citizens in another country and the year of leaving Montenegro.
Article 4
The form of the request for issuing travel certificates (Form number 3) shall contain the name and the location of a diplomatic or a consular representative office of Montenegro that the request is to be submitted to, a box for a 35 x 45 mm photograph, number of the request (bar code), the name: “REQUEST FOR ISSUING TRAVEL SHEETS” and places for entering the data referred to in Article 29, paragraph 1 of the Law, and the data on the language and the script used for entering the data on personal name, the country and address of Montenegrin citizens in another country, the date and the reasons for going to another country and a place for signature of parents and/or legal representatives.
The Form referred to in paragraph 1 of this Article shall also contain the note: “Citizens must enter correct information in the request” and “The Request is to be filled in n readable capital letters”.
Article 5
The form of the certificate of submitted requests for issuing travel documents (Form number 4) shall contain the name of the authority that issued the certificate, the number of the request for issuing travel documents (bar code), the name: “CERTIFICATE OF SUBMITTED REQUESTS” and places for the last and the first names of Applicants, personal identification number, the date of submitting the request for issuing travel documents, the date when the travel documents or the decision on the request may be collected, notification for the Applicant which evidence he/she is obliged to submit and by when, and the signature of authorized Officer.
Article 6
The Form for reporting on disappeared travel documents (Form number 5) shall contain the name of the authority that the request was submitted to, the number of the report (bar code), the name: “REPORT ON MISSING TRAVEL DOCUMENTS” and places for the last and the first name of Applicants and the name of parents, personal identification number, residence and the address of the residence, the date of disappearing of travel documents, a statement whether travel documents were lost or stolen, serial number of travel documents, type of travel documents, the place and the date of issue, signature of the person who filed the report and signature of parents and/or legal representatives.
The Form referred to in paragraph 1 of this Article shall also contain the notes: “Citizens must enter correct information in the Report” and “The Report is to be filled in readable capital letters”.
Article 7
The form of the consent of parents or legal representatives for travelling to another country (Form number 6) shall contain the name: “CONSENT”, places for: the last and the first names of parents and/or legal representative or the name of the custody authority, the note: “WE APPROVE, YES”, places for the last and the first names of minors or other work unable person, personal identification number, date of birth, gender, place and the country of birth, residence and address of the residence, passport number, the date and the place of issue, the text: “MAY TRAVEL TO ANOTHER COUNTRY ACCOMPANIED BY”, places for the last and the first names of persons accompanying minors or other work unable person, personal identification number, date of birth, gender, place of birth, residence and address of residence, passport number, the date and the place of issue, signatures of parents and/or legal representatives, the name of the authority that verifies the consent and the signature of authorized Officer.
Article 8
Forms no. 1, 2, 3, 4, 5 and 6 shall be printed and enclosed to this Regulation and be an integral part thereof.
Article 9
This Regulation shall come into effect on the eight day from its publishing in the “Official Journal of Montenegro”.
