Job Title: Dementia Care Specialist
Job Summary: Provides personal care, emotional support and meaningful activities to residents with dementia, utilizing specialized Dementia Care skills and personal knowledge of the individual resident.
Reports To:Residence Manager
FLSA Status:Non-Exempt
ESSENTIAL FUNCTIONS of the job:
- Develop and maintain close professional relationships with residents affected by dementia;
 - Facilitate interactions among residents and between residents and visitors to the house,
 - Assists to maintain resident independence by learning their capacities and supporting them to do for themselves what they are able;
 - Able to communicateeffectivelywith staff, clients, client family members, community members and clients health care providers;
 - Must be able to communicate fluently in spoken and written English; take and understand direction from RN and supervisor and document care as required;
 - Demonstrates accurate administration of medication, documents required healthcare tasks in accordance to the WAC and Full Life Care policy;
 - Determines clients’ situation and needs by establishing rapport with residents; collaborating with resident, caregiver and team members to establish service goals and plans
 - Addresses client personal care needs by assisting clients in restroom, with ambulation, dressing, meals, bathing and hygiene as needed and as directed in the written Care Plan.
 - Monitors for changes in resident’s behavior, physical health, and mental statusand responds to them appropriately
 - Prepares and serves meals according to planned menu;
 - Performs housekeeping duties as needed or assigned,
 - Maintains continuity of care by communicating changes in client condition verbally and in writing to team members
 - Maintains a safe and healthy environmentby adhering to infection control protocols; addressing potential safety and health hazards; following emergency policy and procedures; and fulfilling education and certification requirements;
 - Promotes client independence, education and recreation by developing and facilitating culturally relevant and cognitively appropriate activities; encouraging interaction and participation;
 - Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner
 
MARGINAL FUNCTIONS of the job:
- Maintains personal care supplies inventory by checking stock level; anticipating needs; informing appropriate team member when supplies need to be ordered;
 - Complete projects and tasks as assigned by supervisor;
 
Educational Requirements Needed to Perform the Duties of the Job:
- Must have a high school diploma or equivalent, and two or more years of experience working in a health or social services setting, college experience is preferred;
 - Must have or acquire and maintain NAC or NAR designation and complete all required Boarding Home training courses in addition to obtaining Nursing Delegation certification;
 - Must have valid Food Handler’s permit, CPR, certificate, First Aid certificate;
 
Physical Requirements Needed to Perform the Duties of the Job:
- Must be able to lift 25 lbs. and support clients’ mobility;
 - Must submit to PPD skin test or provide proof of negative chest x-ray
