1. Save the attached 2 page document (booking form Limerick conferenceHTAI.doc) to your computer.
2. Open the document.
3. Type your personal details on the form.
4. In the boxes on the right hand side of the form, select the workshops you wish to attend in order of preference by typing 1,2,3,4,5 etc.
5. Save the file with the details included.
6. Create a new email and attach the document with your completed form to it.
7. In the subject box of the email, type the following:
