This post, recognised by the SAC in Orthodontics, builds on the initial three year training programme and equips trainees for the ISFE in Orthodontics and provides the preparation for Consultant appointment.
The requirements of this programme are such that the timetable included with the Job Description is to be regarded as provisional and flexible.
Every attempt will be made to adapt the programme to the needs of the individual trainee so that an appropriate balance of experience can be successfully achieved over the two year training period.
THE HOSPITALS AND DEPARTMENTS INVOLVED IN TRAINING IN ORTHODONTICS
BIRMINGHAM DENTAL HOSPITALBirmingham is a thriving city with a population approaching one million, and forming part of a larger conurbation of almost 2.5million. The city has excellent road and rail links and its airport is currently the fastest growing in the country. A host of social, cultural and sporting activities and entertainment are readily available within easy travelling distance. The International Convention Centre is the largest in Europe and includes a concert hall for the City of Birmingham Symphony Orchestra. Nearby are the National Indoor Arena, Town Hall, Museum and Art Gallery and the City Library, itself one of the largest in Europe. There are three professional theatres in the city and Stratford-upon-Avon is only 30 miles distance. On the outskirts of the city is the National Exhibition Centre which, besides being the regular home of many large national and international exhibitions, is also the venue for an increasing number of sports and entertainment events of note.
THE DENTAL HOSPITAL
The Dental Hospital shares a purpose built facility in the centre of the City with the University of Birmingham School of Dentistry and is adjacent to Birmingham Children’s Hospital. The Dental Hospital provides an out-patient and day-case service with approximately 130,000 patient attendances per annum, of which some 20,000 are new patient registrations.
All the dental specialties are represented within the building by consultant clinics and University teaching departments. Excellent liaison exists with clinical and laboratory departments within local Hospitals and a comprehensive range of investigative and reporting services is available to the site.
The Dental Hospital is a Clinical Directorate constituting one of the specialist services of Birmingham Community Health Care Trust, the largest PCT in the country.
It is proposed that the dental hospital will move to a brand new building located at Pebble Mill in the autumn of 2015. This move will affect the successful applicant to this post.
THE ORTHODONTIC DEPARTMENT
The Orthodontic Department is located on the Third Floor of the Dental Hospital, adjacent to the Department of Paediatric Dentistry. The facilities are shared with the School of Dentistry for undergraduate teaching.
There are 14 well-equipped dental units, including an enclosed surgery, a large demonstration surgery, and a central administration area. Also within the department is a tutorial room, a dedicated computer facility and a record model store. The Orthodontic Laboratory is located adjacent to the Clinical Department, with an establishment of five technicians. The department has a 1 WTE Senior Dental Nurse and 6.6 WTE Dental Nurses plus 3.0 WTE trainee Dental Nurses. There is full secretarial support.

The department runs a 3 year M.Orth/MPhil training programme, with an annual intake of up to 4 Specialist Registrars.
The Orthodontic Department accepts referrals from Birmingham and the West Midlands. In 2005-2006 there were some 11,500 patient attendances and approaching 1,500 new patient referrals were received.
There is a close working relationship between the Orthodontic Department and the Oral and Maxillofacial Surgery units in Birmingham. Joint planning and review clinics take place regularly at Birmingham Dental Hospital with Orthognathic Surgery being carried out at the OMFS hub at University Hospital Birmingham Trust.

THE WEST MIDLANDS REGIONAL CLEFT LIP AND PALATE SERVICEThe West Midlands Regional Cleft Lip and Palate Service was the first to be designated in England and Wales as part of the reforms following the recommendations contained in the Clinical Standards Advisory Group’s report (CSAG February 1998). The multi-disciplinary team works across a twin-site centre to give a comprehensive cleft service both to children and adults – with Birmingham Children’s Hospital acting as the paediatric hub and Queen Elizabeth Hospital acting as the adult hub.
Orthodontic treatment services are provided at Birmingham Dental Hospital, which is closely situated to Birmingham Children’s Hospital and is the spoke service with the major orthodontic component within the Region, as well as at other Regional Orthodontic units. Mr Lars Enocson is the lead Cleft Orthodontic Consultant.

KIDDERMINSTER HOSPITAL
Kidderminster Treatment Centre provides a total of four operating theatres with 2 intervention rooms. GP beds are available as are beds for overnight stay on a flexible basis according to surgical case mix. The Treatment Centre provides day-case, short-stay elective treatment with diagnostic facilities and a local MIU. Rehabilitation intermediate care is also provided. A satellite renal dialysis unit opened in February 2002 with outpatient services delivered at Kidderminster across all the major specialties.
ALEXANDRA HOSPITAL, REDDITCH
The Alexandra Hospital opened in 1986 on the borders of Worcestershire and Warwickshire, providing 360 day and acute care beds and serving a population of approximately 200,000. Urology and orthopaedic centres for the county are based in Redditch. The Alexandra Hospital has cancer unit status for breast, lung, urology, gynaecology and colo-rectal cancers and dermatology. The Radiology Department is equipped with state of art the MRI and CT facilities.
WORCESTERSHIRE ROYAL HOSPITAL
The Worcestershire Royal Hospital (550 beds) opened to patients in March 2002 and serves a population of approximately 220,000, housed mainly within a ‘state of the art’ building whilst additional buildings provide accommodation for ophthalmology, care of the elderly, rheumatology and the Charles Hasting Education Centre. The hospital has nine operating theatres including four ‘laminar’ theatres.
The facilities for medicine include a combination of a medical assessment unit, short stay and acute medical beds, six bedded stroke unit and an eight bedded coronary care unit with extensive critical care facilities.
Management Structure of the Trust
There is a strong commitment to clinical leadership within the Trust with a Trust Operational Board and five county-wide Divisions namely Medicine, Surgery, Women and Childrens, Ambulatory Care and Anaesthetics, and Clinical Support. Each Division has a Divisional Medical Director and Clinical Directors in each specialty area. The Trust works closely with the Local Negotiating Committee (LNC) which is appointed by the Medical Staff Committee (MSC).
GENERAL BACKGROUND
Worcestershire is a largely rural County, with four main towns, Bromsgrove, Kidderminster, Redditch and Worcester, lying between the Cotswolds, Welsh Border and Birmingham. There are four further substantial market towns of Malvern, Pershore, Evesham and Droitwich. Acute medical services are provided by two District General Hospitals, a Treatment Centre and a number of Community hospitals.
The major motorway network of the M42, M5 and M6, providing excellent communications, intersect the county. Local leisure facilities are well developed and the area has easy access to Birmingham/Solihull where the National Indoor Arena, International Convention Centre with the Birmingham Symphony Hall and the National Exhibition Centre are located. Stratford and the Royal Shakespeare Company are on the doorstep. Worcester City lying on the banks of the River Severn has the world famous cathedral, County Cricket Ground, Premiership Rugby and is home to the Royal Worcester Porcelain Factory. The Malvern Hills and the Wyre Forest are in the locality.
Worcestershire has many sites of historical interest and natural beauty. The economy of the county is based on agriculture, horticulture, light engineering and increasingly service industries.

In addition, there are two Higher Specialist Trainees in Oral and Maxillofacial Surgery, 2 Specialist Registrars in Orthodontics, 4 Senior House Officers in Oral and Maxillofacial Surgery, 2 Specialist Orthodontists and 1 Orthodontic Clinical Assistant.
The Department also benefits from a comprehensive Orthodontic and Maxillofacial Laboratory at both Kidderminster and Worcester sites with 4 technicians and 1 trainee technician located alongside the main clinical departments. The department also has Dolphin Imaging fully networked across sites and full facilities for orthognathic planning.
The Orthodontic Consultants accept referrals from the whole Worcestershire area with a high caseload at both sites.
Joint Clinics for facial deformity patients from both sites are now centralised at Kidderminster Hospital and joint Oral Surgery / Orthodontic patients all go through a multidisciplinary joint clinic pathway.

SPECIALIST REGISTRAR

POST CCST TRAINING APPOINTMENT
TRAINING PROGRAMME
The Fixed Term Training Appointment in Orthodontics is for an additional two years following on from the 3 year UK specialty programme. It is structured to equip trainees to meet the regulations for the Intercollegiate Specialty Board Examination in Orthodontics (ISFE) of the Royal Colleges and to provide eligibility for appointment to a Consultant post in Orthodontics in the National Health Service.
This post is based at a major Dental Hospital and the trainee is involved in the service and the undergraduate and postgraduate teaching activities of a Hospital and University Orthodontic department.
In addition, access is provided to all dental specialties with established joint working arrangements and with an emphasis on a multi-disciplinary approach. The trainee will be an integral member of the team providing the orthognathic component in the joint assessment, planning and treatment of patients requiring orthognathic surgery by the Birmingham Oral and Maxillofacial Surgical Consultants, whose in-patient hub is centred at the Queen Elizabeth Hospital.
The trainee will be a member of the West Midlands Regional Cleft Lip and Palate Team based at Birmingham Children’s Hospital. The orthodontic management of the cleft patients is undertaken within the orthodontic department of the dental school. The trainee will also attend clinics of the Supra-Regional Cranio-facial Team.
Up to four sessions each week are spent at the regional Orthodontic Unit at Kidderminster Hospital where the trainee is a member of the clinical team involved with the Consultants in Oral and Maxillofacial Surgery in the management and treatment of patients requiring elective Orthognathic Surgery.
At both centres expertise and training will be provided throughout the training period to develop appropriate management skills. Study time is made available and further research and publications are encouraged.
SPECIALIST REGISTRAR POST CCST TRAINING APPOINTMENT
6.0 AIM OF THE POST CCST TRAINING APPOINTMENT
6.1 To prepare for a Consultant post and for the ISFE in Orthodontics held by the Royal Colleges.
7.0 OBJECTIVES OF THE SPECIALIST REGISTRAR
FIXED TERM TRAINING APPOINTMENT
7.1 To provide an appropriate clinical training in the procedures of Orthodontic diagnosis, treatment planning and case management
7.2 To gain experience in the joint management and treatment of cases requiring elective surgery, including Orthognathic Surgery
7.3 To participate in the team approach to the care and treatment of patients with cleft of lip and palate
7.4 To develop expertise in the assessment and treatment of problems requiring inter-disciplinary dental specialty management
7.5 To gain expertise in the assessment and appropriate case management of patients whose Orthodontic treatment is to be undertaken outside of the Hospital service
7.6 To gain experience of and be involved in Orthodontic training at undergraduate and graduate levels, both specialist and non-specialist
7.7 To continue to develop teaching and communication skills
7.8 To gain insight into issues relevant to the provision of Orthodontic services, to be exposed to management issues, and to develop appropriate management skills
7.9 Through involvement in continuing professional education, clinical audit and peer review to develop the skills of critical self appraisal in the delivery of patient care
8.0 GENERAL CONSIDERATIONS
8.1 The period of training will follow completion of three year specialty training and the acquisition of a Membership in Orthodontics of the Royal College of Surgeons, or equivalent
8.2 The training programme will normally be two years’ duration
8.3 During this training period the Specialist Registrar will be expected to publish research and observations of clinical importance
4.0 FACILITIES AVAILABLE TO THE SPECIALIST REGISTRAR AT THE DENTAL HOSPITAL
4.1 The Specialist Registrar will work in a modern, fully-equipped clinical department with appropriate supporting facilities and staff including Radiography and chairside and secretarial assistance
4.2 A dedicated Orthodontic Laboratory adjacent to the department provides full technical support
4.3 The services of a department of Clinical Illustration and a University department of Medical Photography are readily available
