1. Call To Order
The Colonial Regional Local Human Rights Committee was called to order at 5:30 p.m. January 15th 2015, by Denise Gillis; for the purpose of hearing business pertaining to the agenda, transactions, or other matters that may be presented to the committee.
2. Review and Approval of Agenda
January 15th 2015 Agenda was reviewed.
Action: Ms. Clemmons made a motion to approve the agenda as presented

3. Review and Approval of Minutes
October 16th 2014 Minutes were reviewed.
Action: Ms. Clemmons made a motion to approve the agenda as amended

4. Announcements

5. Public Comment
A questions was asked about the relationship between logisticare and the office of Human Rights. Mr. Jones clarified that Logisticare is not licensed by the office of Human Rights.
6. Human Rights Advocate Comments – Mr. Jones again reminded providers that the quarterly reports need to be sent via mail versus email. Even if provider does not have clients, they still need to submit a quarterly report. If a provider is affiliated with the LHRC they need to submit the quarterly report. Quarterly reports need to be sent out two weeks in advance of the meeting.
Mr. Jones also notified the affiliates that they need to secure a professional as an applicant for the LHRC.
Every facility that is licensed will need to submit their annual seclusion and restraint report (January 1, 2014 - December 31, 2014), by January 15, 2015. The report needs to be submitted (mail, fax or email) to the below individual and not to the OHR.

a. The Peninsula Community Care Center, LLC requests approval of its behavioral management policy
Action: Ms. Clemmons made a motion to approve the policies
Second: Ms. Reid
Vote: Unanimous
b. The Empowerment Network, LLC requests approval of its behavioral management policy– Najmah Abdullah, CEO
Action: This item was tabled until the next scheduled meeting.
c. LEVOC – Complaint report to be discussed in closed session

10. Closed Session – The committee voted to go into closed session per VA Code 2.2-3711(A), for the purposes of hearing information pertaining to the complaints/allegations against Martha’s Place.
The Committee voted to come out of closed session and each committee member certified that the only information discussed was the complaints/allegation against Martha’s Place:

In the event of future injuries Martha’s Place needs to ensure that they receive discharge documentation from the hospital so they can ensure a continuum of care.
