Interview Preparation Form

This formis designed to help the Evaluation Team prepare for and conduct an effective applicant interview. There may not be enough time to cover everything the team would like to address, so it is essential to identify priorities and ensure adequate time to address those priorities in depth. The capacity interview is intended to 1) probe questions or concerns raised by the application; and 2) evaluate the capacity of the applicant group to successfully implement the plan.

INSTRUCTIONS FOR TEAM LEADS: Please complete this form prior to travelling for your interviews and distribute to all evaluation team members. You will have an opportunity to review and further organize this information as a group the evening before your interviews, to assign questions to individual members, etc.

INSTRUCTIONS FOR ALL EVALUATORS: Please review this form prior to the team preparation meeting in Washington. Be prepared to discuss the questions and priorities with your team.

PREP MEETING: The primary goal of the preparation meeting is to review priorities, assign questions to team members, and ensure that questions are sufficiently detailed. Do not plan to significantly revise your forms, or leave significant details/amounts of writing to be handled at the meeting.
