Terms of Reference

Vision

  • In 2020 we will work together better to enable the people of Hull to improve their own health, resilience, wellbeing and to achieve their aspirations for the future.
  • Incorporating this vision, we want everyone to have the same opportunity to feel safe, independent and fulfilled no matter what his or her circumstances.

Principles of Partnership Working

  • Participant organisations will work collaboratively and support each other over the period of transformation
  • Participants will do all things reasonably expected of them by the others to give effect to the spirit and intent of the Hull 2020 Programme and will not impede or restrict each other's performance of the Activities
  • Participants will ensure the Hull 2020 strategy is aligned with their own organisations strategies and business development plans
  • Participants will be open, transparent and act in good faith to each other
  • Participants will be of equal status and standing within the programme
  • Participants will work in the interests of the population of Hull rather than current organisations. All decisions will be made on a 'best for Hull 2020 project' basis
  • Participants will commit their resources appropriately to support a timely delivery of the agreed programme objectives
  • Open, straight and honest communication, understanding and respecting other people's perspectives within the Participants and with all key stakeholders. As far as possible commit to try and resolve all issues within the Participants
  • All transactions relating to Hull 2020 are fully open book. The Project will work with clear responsibility and accountability, without blame or surprises
  • Recognition that this is an iterative programme
  • Partners work to ensure services are integrated, high quality and accessible in ways to offer people appropriate choices
  • Partners work to promote self care, reablement or mutual support in community settings so this is viewed as the norm and reduces reliance on residential and care homes
  • Partners work to ensure timely and appropriate support to families and carers of all ages
  • Partners ensure that individuals can plan their care, organised around and within communities, with people who work together and have a shared understanding of the person’s physical and emotional needs
  • Partners encourage a culture across organisations that celebrates diversity, encourages respect and has zero tolerance on hate crimes
  • Participants will use their influence to support the Hull 2020 programme and the Joint Health and Wellbeing Strategy

Role & Purpose

The Safe and Independent Lives Group has a critical role within the Hull 2020 Programme, enabling implementation of the Principles of Delivery and all Safe and Independentrelated initiatives. Because of the extensive shared remit, the group also has a key role in overseeing the delivery of the aims under Outcome 3 (“Safe and Independent Lives”) of Hull’s Joint Health and Wellbeing Strategy 2014-2020.

  • Support of the overall development and implementation of the Hull 2020 Strategy and Outcome 3 of the Health and Wellbeing Strategy (with the exception of the ‘fuel, food and finance’ initiatives, which are the remit of the Health Inequalities Group and the ‘resilience to severe events’ elements which are covered by the remit of the Systems Resilience Group and the Community Hubs Design and Implementation Group.)
  • The initial focus is on vulnerable groups, particularly (but not limited to) the frail elderly. However, the emphasis may change as the work plan progresses.
  • Identification of risks and opportunities, and appropriate mitigation of those risks; ensuring that the Programme Delivery Board and the Health and Wellbeing Board is appropriately briefed on progress, risk and issues
  • Defining the scope of the Work Stream, under agreed broad headings that include the Better Care Fund, and develop an annual project plan to support the delivery of the defined scope.
  • Production of relevant strategies that support the objectives of the Work Stream and the overall Programme
  • To ensure alignment with other local and national initiatives
  • Awareness of similar programmes in surrounding geographical areas, including identification of potential synergies
  • Proactively pursuing new ways of working and identify best practice
  • To invite other strategic leads to meetings to make recommendations to it, in order to link together common issues and implications for other developments taking place across the City
  • Prioritise Work Stream initiatives and ensure clear milestones are defined
  • Ensure Governance arrangements are in place for the Work Stream and all individual initiatives
  • Provide assurance to the 2020 Programme Delivery Board and the Health and Wellbeing Board that all individual projects within the Work Stream are on track and that expected outcomes and benefits are realised

Responsibilities

The responsibilities of the Safe and Independent Lives Group fall into two broad areas (see p4):

(i) Oversight of reporting against activities that contribute to ‘Frailty & Isolation’ initiatives and ‘Safe and Independent Lives’ (Outcome 3 of the HWBS 2014-2020):

  • More people able to manage their own health and wellbeing
  • More people living safely and independently in the community (with particular focus on vulnerable groups) and more people in control of their needs.

(ii) To create a Provider Forum for oversight ofagreed delivery aspects of the:

  • Better Care Fund
  • Extra Care Board
  • Adults Safeguarding Board
  • Learning Disability Partnership Board
  • End of Life Group

Accountability & Reporting

  • The Safe and Independent Lives Group will be responsible for developing, delivering and monitoring its own work programme
  • It will be accountable to the Hull 2020 Programme Delivery Board and the Health and Wellbeing Board
  • It will identify the requirements for each of the Safe and Independent project task and finish groups
  • It will agree / allocate appropriate professionals to the Group and its project task and finish groups, who will act as a conduit between these groups and the Safe and Independent Lives Group itself.

The Safe and Independent Lives Group reports through the respective organisational governance arrangements (see p7 8) through to the Hull 2020 Programme Board and the Health and Wellbeing Board.

Members:

  • Members will demonstrate a commitment to consult this wider network on the work of the group, to attend meetings and complete allocated work within the agreed timescales. If members are unable to attend, they will ensure that they are represented by an appropriateindividual to deputise.
  • Members of the Safe and Independent Lives Group will draw on their wider network of professionals, within their own organisation and the health, social care and public sector system.

Quorum:

As the Group advises the Hull 2020 Programme Board and the Health and Wellbeing Board the issue of quoracy is not critical but it is expected that at least 4 members from separate organisations will be present at each meeting, including the Chair or Deputy Chair.

Meeting Administration and Project support:

Meetings will be scheduled Bi Monthly throughout 2016. The agenda and papers will be produced 5 working days in advance of the meeting. The meetings will be administered and project support will be provided by City Health Care Partnership CIC

Declarations of Interest:

In relation to any item on meeting agendas members are reminded of the need to declare:

•any interests which are relevant or material to Hull 2020 Partnership Organisations

•any changes in interest previously declared

•any pecuniary interest (direct or indirect) on any item on the agenda

Any declaration of interest should be brought to the attention of the Chair in advance of the meeting or as soon as they become apparent in the meeting. For any interest declared the minutes of the meeting must record:

•the name of the person declaring the interest

•the agenda item number to which the interest relate

•the nature of the interest

and be declared under this section and at the top of the agenda item to which it relates.

Review:

The terms of reference will be reviewed at least annually.
