G.PurposeoftheResidency-Our goal is to develop a pharmacy leader who is professional and ethical and who can function autonomously as an integral member of the health care team in the treatment and care of patients with infectious diseases. The resident will become an effective, competent infectious diseases pharmacist in the hospital setting. The hospital based ID pharmacist will be competent to function as a rounding pharmacist on the infectious diseases team, will be able to make recommendations in compliance with effective antimicrobial stewardship, will be able to function as a practitioner in a hepatitis C and HIV clinic.

H.ResidentAcceptance-Uponacceptanceintotheprogramtheresidentwillbeinformedinwritingofthetermsandconditionsoftheappointmentandprovideasignedcopyofacceptanceofsame,includingthe completion of a PGY1 residencycurrentPGY2 Infectious Diseases -residentmanual.

I.ResidentEvaluation

Theresidentwillbeperiodically(butatleastquarterly)evaluatedbytheresidencyprogramdirectorusingpersonalobservationsandcommentsfromtheotherpharmacistpreceptorsand/orhealthcareprofessionals.Observationsarebasedonadherencetoandcompletionofthegoalsandobjectivesasestablishedintheresidency handbook andasdiscussedwiththeresidentduringroutineevaluations.

Necessaryimprovementswillbedocumentedandexpectedtoberesolvedwithin30days.Relativecommentsonsummativeordevelopmentplanswheretheresidentisinneedofimprovementtoachieveaspecificgoalwillbecommunicatedfrompreceptor(orRPD)tothenextrotationpreceptorverballyorinwriting.

Allevaluations(bothcompletedbytheresidentandpreceptor)shouldbecompletedusingPharmacademicTMwithin 7 days from the end of the rotation,ideallypriortotheconclusionoftherotation.Itistheresponsibilityofthepreceptortocompleteanevaluationoftheresidentandreviewthisevaluationwiththeresidentattheconclusionoftherotationinordertoprovideconstructivefeedback. Theresidentandpreceptorshouldreviewevaluationstogether.

Atthebeginningof the residency andatleastquarterlytheresidentandRPDwillcompleteacustomizedtrainingplanaddressingatleast:

-ThefollowingactionplanfortheresidenthasbeendevelopedbasedonthegoalsandrequirementsoftheIDPharmacyResidencyProgram. Itincorporatesanevaluationoftheresidentspastexperiences,strengthsandweaknesses,andtheresident’sindividualgoalsandinterests. AttheendoftheresidencytheResidentandRPDwillcompletethroughthecustomizedplan,theyearendevaluation.

-Ifselfassessment(reflectiveinquiry)isnotnotedtheRPDmayreturntheevaluationforimprovement.

J.DefinitionsforPharmacademicevaluations.

For regular rotations

•ACH- Resident has demonstrated independence in this area or has refined judgment to ask for advice. For example, the resident should be able to write notes, order labs and cultures independently with little or no guidance from the preceptor

•SP- Resident is able to ask questions to acknowledge limitations and/or judgment is not refined.

•NI- Resident is unable to ask appropriate questions to supplement limitations and/or has a general deficit in this area.

For advanced rotations: (Advanced Stewardship and ID Consult Service II)

•ACH- Resident should work independent of the preceptor. The resident shall be able to communicate to the teams and answer questions without running by each plan with the preceptor

•SP- Resident asks questions to acknowledge limitations.

•NI- Resident is unable to ask appropriate questions to supplement limitations and/or has a general deficit in this area.

Programassessment-Periodically,butnolessthanannuallytheResidencyCommitteewiththeprogramdirector,willreviewPharmacademicProgramandPreceptorevaluationsandpresentanyfindingstothegroupfordiscussion/change.

K.Preceptor,RotationandProgramEvaluation

Theresidentistoevaluateboththepharmacistpreceptors,non-pharmacistpreceptorsandtheindividualclinicalassignmentfollowingcompletionofhis/hertourofduty(butnotlessthanquarterly).TheevaluationwillbediscussedwiththeindividualpreceptorsandPharmacyServiceManagerasrequired.Onanongoingbasis,theresidentprogramdirector(RPD)andresidencycommitteewillevaluatetheprogramaswellastheresidentprogress.Whenneededbutnolessthanonceper yeartheRPDwillconveneameetingofvariouspreceptors/residencycommitteetoevaluateprogramstructureandcontent.

L.Attitude/Professionalism

Theresidentisexpectedtodemonstrateprofessionalresponsibility,dedication,motivation,andmaturitywithregardstoallactivitiesandresponsibilitiesassociatedwiththeresidencyforitsentirety.Theresidentshalldemonstratetheabilitytoworkandinteractwith thestaffandpatientsoftheMedicalCenterinaproductiveandharmoniousmanner.Appropriateattire,personalhygieneandconductareexpectedatalltimes.TheresidentwilladheretoalltheregulationsgoverningtheoperationsoftheDepartmentofVeteransAffairsMedicalCenterwithoutexception.

M.Attendance/Leave/TerminationVacation/Sick-leave/Holidays

  • Allrequestsfortimeoffincludingvacationandholidaysmustbepre-approvedbytheRPD,rotationpreceptor,PharmacyServiceManagerordesigneeandfollowVAproceduresforrequestingleaveandbegivenwithadequatenotice(minimumof2weeks).Iftheseprocessesarenotfollowedtheabsencewillbeconsideredunexcused.
  • Promptarrivalandattendanceisrequiredatallclinics,conferences,meetings,roundsandotherscheduledactivitiesduringeachandeveryrotationthroughoutthetermoftheresidency.
  • Theresidentisresponsibleforreschedulingorarrangingalternatecoverageforallactivitieswhichwilloccurduringanyplannedabsences
  • Unexcusedabsencesandortardinesswillnotbetoleratedandcanbeabasisforfailureoftherotationinvolved.
  • ItistheresponsibilityoftheresidenttocontactthepreceptororthepharmacyAOassoonasispracticaltoreportunavoidableabsencesortardiness.

N.Extended Absence

  • Intheeventofanextendedabsenceduringtheresidency,theRPDwillcoordinatewithHumanResources(andifneededtheVAOfficeofAcademicAffiliations-OAA)arrangementsneededtocompletetheresidency.
  • Thetermoftheresidencyshallbeconsidered2080hoursandtheprogramwillnotbeconsideredcompleteuntilboththehoursandrequirementsaremet.
  • ThePharmacyManagerwillbenotifiedofanyextendedabsenceduringtheresidency.

O.Termination/Resignation

  • Intheeventthattheresidentchoosesnottocontinuetheresidencyandmeetthestatedrequirements(hoursandlearningobjectives)heorshewillberecommendedforterminationbytheResidencyCommitteetotheChiefPharmacyService.Alternatively,theresidentcanchoosetosubmitaletterofresignation.

P.Professionalmeetings:

  • TheresidentisencouragedtoattendvariousprofessionalmeetingssuchasID Week and ASHPMidyearClinical
  • Whenleaveandpossiblefundingisinvolved,theresidentwillcomplywithVHAWNYandVISN2travelpolicies
  • Neitherleavenorfundingisguaranteed
  • Beingapresenter,lecturerincreasesthelikelihoodofsomesupport

Q.Grievances

Anyproblemthatmayariseduringtheresidencyshouldfirstbedealtwithbytheappropriatepreceptor.Iftheattemptstoresolvetheproblemareunsuccessful,itshouldbebroughttotheattentionoftheProgramDirector.Ifforsomereasonitisunabletoberesolvedatthatlevel,thePharmacyServiceManagerwillhavetheauthoritytomakethefinaldecision.

R.TerminationPolicy

Aresidentmaybeterminated,upontherecommendationoftheResidencyCommittee,atthediscretionofthePharmacyServiceManagerforthefailuretomeetprogramgoals/objectivesorplanneddurationasoutlinedinthistextorforfailuretomeetthetermsofemploymentoftheVAWesternNewYorkHealthcaresystemfacilities.ThesecouldincludeexcessiveAWOLpractices,substanceabuse,mentalimpairment,harassment,theftofgovernmentpropertyand/orinappropriateprofessionalconduct.AnyterminationwillfollowstandardVAHRpracticesofprogressivediscipline.VHAhandbook5005(hiring)andVHAhandbook5021(termination)

A residentmayalso beconsideredforterminationiflicensureisnotobtainedattheearliestpossibletime(e.gSeptember1st)butnolaterthanSeptember30thoftherespectiveyear.GrievanceswillbemanagedasnotedinitemGabove.

S.Pharmacademic.–AllresidentswillutilizePharmacademic to complete evaluations and to upload residency documents.

UsingPharmademicwillaccomplishtheCustomizedResidencyPlan,Formativeevaluations,SummativeEvaluationsandPreceptorEvaluations.PrintingandplacingPharmacademicdocumentsintheresidentbinderisoptional.

T.Graduation – In order to successfully complete the residency the resident must be present for 80% or more of each scheduled learning experience, attain “achieved (ACH)” in >85% of the residency program specific evaluations of goals and objectives, attain no “needs improvement (NI)”. In addition residents must be licensed, complete a research project, professional presentations, an ACPE accredited CE and all items in the checklist.

U.Scheduleofrotations–Themasterrotationschedulewillbepostedonthe

G:\DT-Pharmacy\ResidencyInfodrive.ThisscheduleshouldbeconsistentwiththePharmacademicandthemasterpharmacistschedule.AnydiscrepanciesshouldbebroughttotheattentionoftheRPDortheprogramadministratorimmediatelyupondiscovery.

V.DutyHoursTobeinaccordancewithASHP/ACGMEstandards (see ASHP website) theresidentwillsupplyregularcommunicationdetailingallextramural(moonlighting)hoursworked.Thiswillbedocumentedonthecustomizedresidentplanwiththeresidencyprogramdirector,nolessthanquarterly.

MaximumHoursofWorkperWeek

  • Dutyhoursmustbelimitedto80hoursperweek,averagedover afour-weekperiod,inclusiveofallin-housecallactivitiesandallmoonlighting.
  • Moonlighting(i.e.,workingoutsidetheresidencyprogram)ishighlydiscouraged.
  • Moonlightingmustnotaffecttheresident’sjudgmentwhileonscheduleddutyperiods(asassessedbythepreceptororothersupervisingentity),interferewiththeirabilitytoprovidesafepatientcare(asassessedbythepreceptororothersupervisingentity),orimpairtheirabilitytoachievetheeducationalgoalsandobjectivesoftheirresidencyprogram(asassessedbythepreceptorand/orResidencyProgramDirector(RPD)).
  • ResidentsnotmeetingftherequirementsoftheirresidencyprogramasaresultofmoonlightingwillberequiredtocomplywitharemediationplanoutlinedbytheRPDand,ifnoimprovementisseen,willbesubjecttodismissalfromtheresidencyprogram.
  • Allmoonlightinghoursmustbecountedtowardsthe80-hourmaximumweeklyhourlimit.
  • Ifresidentsmoonlight,theymustsubmittheirhourstotheirRPDonamonthlybasis. Ifthenumberofhoursexceedtheabovelimitwhenaveragedoverafourweekperiod,theresidentwillbeexpectedtoreducethenumberofhourstheyarecommittingtomoonlightingsoastomeetthisrequirement.

MandatoryTime Freeof Duty

  • Residentsmustbescheduledforaminimumofonedayfreeofdutyeveryweek(whenaveragedoverfourweeks).At-homecallcannotbeassignedonthesefreedays.

MaximumDutyPeriodLength

  • Continuousdutyperiodsofresidentsshouldnotexceed16hoursinduration
  • MinimumTimeOffbetweenScheduledDutyPeriods
  • Residentsshouldhave10hours(butmusthaveatleasteighthours)freeofdutybetweenscheduleddutyperiods.

W.ResidentSelection/Recruitment/Promotion-

TheresidencywillbepromotedthroughtheASHPWebsite/VAPublicWebsite/VAWNYPublicWebsiteaswellasvariousnationalmeetings

ResidentapplicantsarerequiredtoapplytotheprogramthroughPhorcas.Ascreeningrubricapprovedbytheresidencycommitteewillbeutilizedtoprescreenapplicantsforonsiteinterviews.ThescreeningrubricwillbeapprovedbytheResidencyCommitteeandremainonfilewiththeProgramDirector.Thetop30(nottoexceed30interviews)scoredapplicantsinPhorcasWebAdmitwillbeselectedandcontactedforanonsiteinterview.TheRPDreservestherighttoextendtheapplicationdeadline,withapprovaloftheresidencycommittee,toservetheneedsoftheapplicationprogram.

Allonsiteinterviewswillbeconductedwithatleasttwomembersoftheresidencycommitteeandonecurrentresident(moreareencouraged).Theonsiteinterviewwillconsistofapresentationbythecandidate,interviewbyagroupoftheResidencyCommitteemembers,tourofthefacilitywithstopstomeetpreceptorsofeachrequiredrotationandwrap-upwiththeProgramDirector.Theinterviewquestionsandscoringgridwillbepre-approvedbytheresidencycommittee.Aftercompletionofthelastinterview,theresidencycommitteeandcurrentresidentswillmeetandconsolidateindividualrankingscoresbeforethefinalrankissubmittedtotheNationalMatchingService.Theresidencycommitteereservestherighttonotrankcandidatedeemedunsuitablefortheprogram.“Scramble”IntheeventofanunmatchedresidencypositiontheapplicantwillapplythroughPhorcasWebAdmit,asperthedatespostedontheNationalMatchingWebsite.TheRPD/ResidencyCommitteereservestherighttowaivetheonsiteinterviewandconductatelephoneinterviewofselectedcandidatesinlieuoftheinpersonvisit.PernationalVArequirements,allpositionsmustbefilledbyApril15oftherespective year,ortheymaybesubjecttotemporarytransfertoanothersite.
