Instructions forAcademic Staff-Teaching/Other Teaching Staff Appointments in FASA:
MODIFYA RECORD
Starting the FASA Transaction
- Go to fasa.stanford.edu
- Select New Transaction
- Select form: Academic Staff – Teaching/Other Teaching Staff
- Select Modify Appointment
- Search for a Candidate: Enter the name of the person being appointed and press “Search.”
- Select a Candidate: Select the correct record and press View.
Modifying an Existing Appointment
- Modify any fields that require updating on the existing FASA form. The area on the right with gray headings contains the current appointment details. Enter data about the new appointment in the area on the left with beige headings. Consult the Instructions for New Appointments or Reappointments for more information about specific fields.
- Attachments: Attach any documents needed for the appointment, such as the candidate c.v., the draft offer letter, any required teaching evaluations, Staff HR Approval Form.
- Comments: Add any comments that may be required for the HSDO Approvers.
- Click Submit for Approval (one of the choices in the gray heading bar at the top of the form). Once processed, you will see the name of the next approver at the top under Current Assignee.
Approving the FASA Form
- Approvals: Originatoradds the Department Chair/Program Director (as Department Approver) and April Jackson (as H&S Dean Office Approver). H&S requires at least one faculty approver, typically the Department Chair. If the Originatoris not the Department/Program Manager, the department may choose if and how they wish to include the Department Manager; this may be as an FYI, or as another Department/Program approver. Department Manager approvals are not required as long as the faculty Chair/Director has approved. Aprilwill add all other required approvers.
Post-Approval
- When the FASA form is fully approved by theH&S Dean’s Office, the Originator will receive an email notification from April Jackson.
- Select View Summary (at the top of the form) to print a copy for your department. HSDOwill keep a copy in the faculty file in the HSDO.
