M.G.L.c.ll9A,s.16(a),asyouhavefailedtomakeconsistentandtimelypaymentstowardyourcourt-orderedchildsupportobligationandcurrentlyowepast-duechildsupport. AcopyoftheDORletternotifyingtheBoardtosuspendyourlicenseandciting statutoryauthorityforitsaction isenclosed.

TheBoard,nsrequiredbylaw,herebynotifies youthatyourrighttorenewyourlicensetopracticeasaPharmacyTechnicianinMassachusettswillbesuspendedeffectivefive(5)daysfromthedateofthisletter. Further,youareherebyrequiredtoreturnany licenseissuedtoyouby theBoard,whethercurrentorexpired,totheBoard'sofficeat239CausewayStreet,Boston,MA02114,by handorbycertifiedmail,withinfive(5)daysofthedateofthisletter.

ThesuspensionofyourlicenserequiresthatyouceaseallpracticeasaPharmacyTechnicianinMassachusetts. Ifyoupracticeinviolationofthislicensesuspension,such

practiceshallbereportedtotheOfficeoftheAttorneyGeneralandyoumayfacecriminalchargesforunlicensed practice.

Ifthereareanycomplaintspendingagainstyourlicense,orifyourlicenseiscurrentlyunderdiscipline,thelicensesuspensionimposedinconnectionwithyourchildsupportdelinquencywillstillbeeffective. ThelicensesuspensionshallcontinueineffectuntiltheBoard receiveswrittennotificationfromtheDORthatthematterofyourchildsupportdelinquencyhasbeenresolvedwithDOR.

Youarefurtheradvisedthatifyoucontinuetofailtopayyourchildsupportarrearageinfull,orifyoufailtocomplywithallcurrentchildsupportpaymentorders,orboth,theDORmayrequesttheBoardtorevokeyourlicense.

YoumayseekjudicialreviewoftheDOR'sdeterminationtosuspendyourlicensebyfilingacomplaintagainsttheDORasstatedinparagraph3oftheenclosedOORnotice.YourappealrightsarefullysetforthinG.L.c.119A,§16(a)(2)(e)(I).Anyappealmustbefiledintheappropriatecourtwithinforty-five(45)daysofthisNoticeofLicenseSuspension.

TheChildSupportEnforcementDivisionoftheDepartmentofRevenue(DOR)basdeterminedthattheindividualidentifiedaboveowespast-duechildsupportthatissubjecttoachildsupportlien,pursuanttoG.I...c.ugA,§6.Youarehereby notified tosuspendthelicenseorcertificateissuedbyyouragencytotheindividualidentifiedabove,pursuanttoG.Lc.119A,§16.

DORaffordedtheindividualanopportunitytosatisfythepast-duechildsupportorrequestanadministrative hearing.TheopportunityforabearingprovidedbyDORistheexclusiveadministrativeremedyforcontestingthedeterminationofachildsupportdelinquency nodforcontestingsuspensionorrevocationofthelicenseorcertificate.G.L.c.119A,§16(b).ThisnoticeisbasedonthefinaldeterminationofchildsupportdelinquencybyDORandisnotsubjecttoreviewbythelicensingauthority.

Theindividualmayseekjudicialreviewinthecourtwherethechildsupportorderwasissued,orwhichbasjurisdictiontoregistertheorder,byfilingacomplaintagainstDORwithin45daysofthedateofthenoticefromyouragencythatthelicenseorcertificatehasbeensuspended.

If,withinsixmonths,theindividualfailstopaythechild supportdebtinfullormakeconsistentchildsupportpayments,DORwillsendyouwrittennoticetorevokethelicenseorcertificate.
